Enclosed are two copies of the Energy Fuels Resources USA Inc. ("EFR[') Corrective Action Comprehensive
Monitoring Evaluation ('.CACME") report for chloroform in perched groundwater at the White Mesa Uranium
Mill (the 'Mill') located near Blanding, Utah. This report is required by Part Itr.H of the Groundwater
Corrective Action Plan ("GCAP") found in Attachment 1, of the final Stipulation and Consent Order ("SCO")
Docket No. UGW2G01, approved on September L4,2015 by the Utah Department of Environmental Quality
