NOTE: 1. Submit this form in conjunction with Form 1 and Form 2.
2. To relocate a permitted Portable Hot Mix Plant submit Form 15b.
3. Call the Division of Air Quality (DAQ) at [phone] if you have problems or questions
in filling out this form. Ask to speak with a New Source Review engineer. We will be glad to help!

1. Indicate the manufacturer of the plant.
2. Indicate the model of the plant.
3. Indicate the maximum capacity of the plant in tons/hour.
4. Indicate the type of fuel used in the plant such as diesel, natural gas or other.
5. Indicate the type of emission control and submit the proper form.
6. Indicate the type of material to be processed through the plant. Either virgin or RAP and the percentage of
each.
7. Indicate the type of power to be used by the plant either line power or portable generator.
8. Supply calculations for all criteria pollutants, greenhouse gases and HAPs. Use AP-42 or manufacturers’
data to complete your calculations
