PST Fund payment claims will reference this work plan and specific tasks itemized below. INTRODUCTION The site is a former gas station and auto shop at 45 North Main Street. The underground storage tanks (USTs) were removed in April 2023. During UST closure activities hydrocarbon concentrations exceeded Initial Screening Levels (ISLs) in a soil sample from the tank basin and exceeded Tier 1 levels in soil samples from three dispensers. The two groundwater samples collected in the tank basin exceeded Tier 1 levels. An initial subsurface investigation defined the extent of soil contamination but not groundwater contamination. The purpose of the additional subsurface investigation is to install groundwater monitoring wells and define the extent of groundwater contamination. SCOPE OF WORK Three monitoring wells currently exist at the site from a previous release that has since been closed. The wells have been renamed MW-1, MW-2, and MW-3 and are shown on Figure 1. Monitoring Well Installation Six groundwater monitoring wells will be installed to a depth of twenty feet and will contain fifteen feet of screen at the bottom. Proposed well locations are shown on Figure 1. Sand will be placed in the boring around and up to approximately one foot above the screen. Each boring will be sealed with bentonite from approximately four feet below grade to the ground surface. The well will be completed with a flush-mount well cover. Each well will be developed, and the elevation of the top of casing will be surveyed for future groundwater depth and flow direction calculations. Work Plan OIB-1, Additional Subsurface Investigation, Main Street Service, Layton, Utah Rockwell Solutions, Inc., 718 E. Bridger Lane, Elk Ridge, UT 84651 801-361-2930 rockwellutah@yahoo.com 2 The wells will be installed using direct push methodology. Soil samples will be collected continuously in five-foot plastic liners to evaluate the soil type and potential soil contamination. A portion of soil from each liner will be collected and placed in a Ziploc baggie and set aside for contaminants to volatilize. The soil in the baggie will be checked with a photoionization detector (PID). Soil samples will be collected for laboratory analysis from the interval with the highest PID reading or, if no PID readings are present, from just above the apparent groundwater table. Soil samples will be analyzed for total petroleum hydrocarbons-gasoline range organics (TPH- GRO), methyl-tert-butyl-ether (MTBE), benzene, toluene, ethyl benzene, total xylenes, and naphthalene (MBTEXN) using EPA Method 8260, and TPH-diesel range organics (TPH-DRO) by EPA Method 8015 by a Utah Certified Laboratory. Groundwater Monitoring At least two days after monitoring well installation, depth to groundwater will be measured and groundwater samples will be collected from all monitoring wells. Sampling will be collected following standard decontamination, sample collection, and sample preparation/ preservation protocol. Groundwater samples will be analyzed for MBTEXN and TPH-GRO using EPA Method 8260 and TPH-DRO using EPA Method 8015. Report A Monitoring Well Installation and Groundwater Monitoring Report will be prepared to document well installation and groundwater monitoring activities including laboratory analytical results. Main Street in Layton, Utah SCOPE OF WORK The following outlines our understanding of the proposed scope of work: ● Drill six (6) borings to 20 feet or refusal using conventional hollow stem auger drilling techniques. ● Sample at 5-foot intervals to the termination depth. ● Install 2-inch diameter monitor wells in each boring. The monitor wells are to be constructed with Sch. 40, PVC casing with 15 feet of 0.010-inch factory slotted screen. The borehole annulus is to be backfilled with 10/20 silica sand from 3 to 20 feet followed with a 2-foot bentonite chip seal to 1 foot below ground surface. Each monitor well is to be completed a 7-inch diameter flush mount well cover set in concrete from the ground surface to 1 foot below ground surface. ● Drill cuttings are to be stockpiled on site. ● Decontaminate the drilling and sampling equipment between the monitor wells. The decontamination water can be released to the ground surface. ● Two of the boring locations are located on concrete pavement and will require coring prior to drilling. The rest of the locations are located in dirt or landscaping. ● All boring locations are accessible to our track mounted drill rig with no overhead restrictions exist at the boring locations.
