Box 144840 Salt Lake City, Utah 84114-4840 RE: Corrective Action Report Maverik Store #289 951 West Highway 40, Vernal, UT 84078 Facility ID# 9000309, UST Release Site MXL Dear Ms. Brennan: Transmitted herewith please find the corrective actions implemented and laboratory analysis results for samples collected during the tank removal activities completed in late November/early December 2023 at the former Maverik store #289 located in Vernal, Utah. UST closure and sampling activities were conducted as per the approved Closure Plan and documented in the UST Removal Report/Closure Notice which was recently submitted to the DERR. Background A release was identified for this site in 2012 and subsequently Release ID MXL was assigned to the site by DERR. Since that time, there have been multiple years’ worth of site investigations and laboratory analytical data. Due to the corrective actions employed in the past, contaminant concentrations had decreased substantially throughout the site. However, as of the last groundwater monitoring event conducted prior to UST removal (conducted on June 26, 2023, there was still one analyte (benzene) that was above the Utah Risk-Based Corrective Action Tier 1 Screening Levels (RBCA Tier 1 SLs) in two wells, MW-2 and MW-4 (see Figure 1). Ms. Eileen Brennan February 2, 2024 Page 2 24-02-02 Vernal 289 proposed CAP report Corrective Actions Store operations ceased on September 7, 2023, and the USTs, dispenser islands, and canopies were removed in mid-November 2023. To facilitate site remediation during the site closure, the following corrective actions were proposed by Maverik, and subsequently approved by the DERR. 1. Soil over-excavation and off-site disposal, 2. Groundwater recovery and off-site disposal, and 3. Application of Klozur One® for chemical oxidation and enhanced bioremediation A Public Notice identifying the above measures was hand delivered to all the neighboring properties with the potential to be impacted on October 27, 2023. Tank removal/corrective action work started on November 15, 2023, and between November 15 and 17, 2023, three (3) existing tanks, four (4) dispenser islands, and associated piping were removed and disposed off-site. Closure Samples Seven (7) closure soil samples were collected for laboratory analysis, just above the water table, at approximately 11 feet below ground surface (bgs) along the walls of the UST basins and four (4) additional soil samples were collected at approximately 3 feet bgs under the removed dispenser islands. Five (5) groundwater samples were collected in the tank excavations at approximately 12 feet bgs. All samples were taken to Chemtech-Ford Analytical Laboratories of Sandy, Utah, for soil and groundwater analyses, following chain-of-custody protocols. Samples were analyzed for benzene, toluene, ethylbenzene, total xylenes, naphthalene (BTEXN), methyl tertiary-butyl ether (MTBE), total petroleum hydrocarbons - gasoline range organics (TPH-GRO) and TPH - diesel range organics (TPH-DRO) according to EPA SW-846 Methods 8260 and 8015. Overexcavation, Groundwater Recovery and Confirmation Sampling Soil was also field analyzed using a MiniRAE 3000+ portable VOC meter/photoionization detector (PID), calibrated to 100 parts per million (ppm) isobutylene standard. A soil sample was placed into a Ms. Eileen Brennan February 2, 2024 Page 3 24-02-02 Vernal 289 proposed CAP report plastic zip lock bag, sealed, and warmed. The sealed bag was opened slightly to insert the tip of the VOC meter to check for the presence of hydrocarbon vapors. PID readings at multiple locations, primarily on the east side of the northernmost tank basin, identified significant vapor readings along with a very noticeable hydrocarbon smell and soil discoloration. As such, over-excavation activities were initiated. The excavation was slowly expanded until clean edges (as evident by relatively low PID readings) appeared to be reached. Confirmation soil samples were then collected at four locations at approximately 11 feet bgs. Confirmation sample locations are shown in Figure 2. A total of 556.5 tons of soil was over-excavated and disposed off-site at the Uintah County Landfill in Vernal, UT (weight tickets attached). In addition, a vacuum truck was mobilized to the site to remove groundwater from the northernmost UST basin. Approximately 2800 gallons of groundwater was extracted into a vacuum truck and taken to Lapoint Recycling in Lapoint, UT for disposal (manifest attached). After which, one additional groundwater sample/confirmation sample (see Figure 2) was taken of the recharged water within the UST basin. Analytical Results Closure Samples Except for samples SS-2 and SS-5, laboratory analytical results for all closure samples were all either non-detect or below initial screening levels (ISLs) for soil. SS-2 samples were above ISLs – but below RBCA Tier 1 SLs - for ethylbenzene, TPH-GRO, and TPH-DRO (at 17.3 mg/kg, 805 mg/kg, and 3140 mg/kg, respectively). SS-5 samples were above the ISL for TPH-DRO (at 1090 mg/kg – see Table 1, attached). Groundwater laboratory analytical results for multiple samples were above ISLs, and a few were above RBCA Tier 1 SLs; for benzene, TPH-GRO and TPH-DRO and, with one exception (GW-3A, toluene @1.75 mg/L), were below ISLs for all other constituents analyzed (see Table 2). Benzene was shown to be above RBCA Tier 1 SLs in samples GW-1 (@ 0.478 mg/L) and GW-3A (@ 0.796 Ms. Eileen Brennan February 2, 2024 Page 4 24-02-02 Vernal 289 proposed CAP report mg/L). TPH-DRO was above RBCA Tier 1 SLs in samples GW-2 (@ 10.2 mg/L) and GW-4 (@ 73.0 mg/L). Confirmation Samples All four soil confirmation samples were either non-detect or below, generally significantly below, ISLs for soil. The one confirmation groundwater sample gathered, identified as GW Confirm, was non- detect for all constituents except TPH-DRO (which was slightly above the ISL @1.2 mg/L). Please refer to Table 3 for confirmation sampling results. Application of Chemical Oxidation As per the approved CAP, prior to backfill with clean imported material, Maverik applied Klozur® One in the tank excavation for chemical oxidation and enhanced bioremediation. As described in the CAP, Klozur® One is composed of soluble sodium persulfate which when dissolved can persist for weeks to months after an application to treat residual contaminants, including hydrocarbon components. Six 55.1 lb. bags of the dry product were mixed with the recommended amount of water (approximately 130 gal. of water/bag of Klozur® One) and liberally applied to the exposed groundwater surface and excavation sidewalls prior to backfill. Installation of New Monitoring Wells Upon completion of the soil over-excavation and recovery of contaminated groundwater, three (3) groundwater monitoring wells (15 feet deep, 4-inch diameter, Schedule 40 PVC, with 10 feet of 0.020 screen) were installed, along the tank excavation. The reasons for installing these three wells were first, to replace wells that were destroyed during the soil over-excavation activities; and second, to use them in the future as a conduit to the tank basin for sampling and/or injection of chemicals such as Klozur® One for chemical oxidation and enhanced bioremediation, if needs be. The approximate locations of these wells are found in Figure 3. Conclusions and Recommendations As evidenced by the laboratory results of the confirmation soil and groundwater samples collected following the corrective actions implemented at the site, we believe that the vast majority, if not all, of the source of contamination is eliminated from the site.
