The primary objective of the corrective actions was to reduce the petroleum constituent concentrations in the direct vicinity of MW-2 and MW-7 to below UDEQ Tier I Screening Levels using the BOS-200® Trap-and-Treat microbial and nutrient technology. In February 2024 a post injection groundwater sampling event was conducted. During the event it was discovered that monitoring wells MW -2 and MW -7 were almost complete filled with the BOS 200 slurry. Upon further discussion with the Utah Department of Environmental Quality (UDEQ) project manager it was decided that MW-2 and MW -7 should be abandoned and new wells installed. Atlas anticipates that the replacement wells will be installed in late May 2024, and the second post-injection groundwater sampling event under WA LXX-20 is tentatively scheduled for early June 2024. Under Work Assignment LXX-21, the UDEQ, Division of Environmental Response and Remediation (DERR) requested Atlas prepare a work plan to install monitoring wells at the above referenced site. A site map depicting the site location and property features is presented as Figure 1. This following proposed work plan presents a brief description of the tasks to be performed including a schedule (Table 1) for completing the activities described herein and a detailed cost breakdown (Table 2). 2.0 SCOPE OF SERVICES The work objective is to provide a work plan and price to install two replacement monitoring wells at the site and prepare a summary report documenting the field effort. Each task is described in greater detail in the following sections. Utah Certified Soil and Groundwater Samplers will perform the sampling activities described below under the direction of a Utah Certified UST Consultant. Atlas will prepare a site specific Health and Safety Plan (HASP) for the scope of work proposed in this document. 2.1 Task 1 – Work Plan Preparation Task 1 is preparation of this work plan. 2.2 Task 2 – Monitoring Well Installation Task 2 will involve the use of a direct-push drill rig to install two monitoring wells (MW -2R and MW -7R) to a depth of approximately 16 feet. Please refer to Figure 1 for the well locations. The wells will be installed to evaluate current groundwater conditions within the chemical injection areas at the site. Prior to the start of the field work, Atlas will contact the Utah Blue Stakes Center and schedule a utility locate for the well installation areas. In addition, a private underground utility locator will be Tesoro #46, WA LXX-21 Atlas 2 utilized to verify that there are no underground utilities present within two feet of the proposed boring locations and each soil boring will be day-lighted to a depth of 5-feet below grade using a hydro vacuum system or a hand auger. During the day-lighting effort hand auger samples will be collected in each soil boring at 2 and 4-feet below grade. During direct push drilling a macro sampler equipped with a nylon sample liner will be utilized to collect continuous soil samples to a maximum depth of approximately 16 feet. All soil samples will be field logged by an experienced geologist. The logging information will include soil type (Unified Soil Classification), color, consistency, moisture content, odor, and organic vapor monitor (OVM) readings. Immediately following retrieval, a composite of each soil sample interval (approximate 2-ft. intervals) will be transferred to laboratory provide jars and to a zip lock type plastic bag. The field portable OVM will be used to screen each bag after adequate time passed for volatilization and capture of the petroleum vapors inside the bags. The field logging observations and OVM data described above will be used to select appropriate samples for chemical analysis. Soil samples, at least three from each borings will be submitted to a State of Utah certified laboratory for MTBE, benzene, toluene, ethyl benzene, xylenes, and naphthalene (MBTEXN) and total petroleum hydrocarbons as gasoline (TPH-GRO) analysis using USEPA Method 8260B, and TPH as diesel (TPH-DRO) using EPA Method 8015B. It is assumed that the soil borings will have more than one soil sample submitted from different zones in an effort to accurately evaluate the vertical extent of impacts. All soil samples will be maintained under chain of custody documentation throughout the sample collection, storage, transportation, and laboratory analysis process. Excess soil cores and decontamination fluid generated during well installation will be thin spread on-site. Upon completion of the well installation effort, MW -2 and MW -7 will fill with bentonite chips and sealed at the surface with concrete. 2.3 Task 3 – Monitoring Well Installation Report Upon completion of the field effort, a Monitoring Well Installation Report will be prepared. The report will include a summary of field activities associated with the well installation advancement; collected soil and groundwater data summarized in table format; soil boring logs; and a brief discussion of the data collected along with conclusions and recommendations based on the presented information. The report will include: • The scope and objectives of the work assignment; • Standard operating procedures sampling; • Soil boring logs and well construction details; • Site map drawn in AutoCAD showing well locations, utilities, structures, property lines, other on-site relevant items, and adjacent property information, • Tabulated soil analytical results; and • Laboratory reports and chain of custody forms. An electronic copy of the report will be provided to the UDEQ and Hardy Enterprises. 3.0 DELIVERABLES The anticipated deliverables for the work identified in this work plan include the following: Tesoro #46, WA LXX-21 Atlas 3 • Monitoring Well Installation Report; and • Invoices, backup documentation, and PST Trust Fund required documents. 4.0 PROPOSED PROJECT SCHEDULE The proposed schedule for the activities described herein is presented in Table 1. 5.0 PERSONNEL The key personnel for the proposed activities are as follows: • Atlas Senior Project Manager: Jim Coletta (P101) • Atlas Professional Geologist: Jim Coletta (P102) • Atlas Project Manager: Jim Coletta (P103) • Atlas Field Scientist: Beau Anderson (P104) 6.0 SUBCONTRACTORS • Direct Push Services will provide the drilling services; and • Chemtech Ford Laboratories will provide state certified laboratory services. Based on information provided by each selected subcontractor, the following contract requirements have been met: • The selected subcontractors are not currently debarred pursuant to Utah Code Section 63- 56-48 or EPA’s most recent “Master List of Debarred or Suspended Facilities”; • The selected subcontractors have no apparent conflict of interest; and • The selected subcontractors are not a minority or women owned business enterprises. 7.0 EXCEPTIONS, ANTICIPATED PROBLEMS, OR SPECIAL REQUIREMENTS At this time, no exceptions or special requirements to the proposed Work Plan are anticipated. In the event exceptions or special conditions arise, Atlas will amend this proposed Work Plan or submit a new Work Plan for DERR approval before initiating any work outside the scope of services described herein. 8.0 CONFLICT OF INTEREST Based on Atlas’s present knowledge of the site location and conditions, no personal or corporate conflict of interest has been identified for the performance of the scope of services described in this proposed Work Plan. Atlas will contact DERR immediately if a potential conflict of interest is identified during the performance of this proposed Work Plan. 9.0 COST ESTIMATE The estimated cost for implementation of this proposed Work Plan is presented in Table 2. The estimated budget will require adjustment if the scope of services identified herein is modified or any assumptions are incorrect. Quotes from drilling contractors were obtained for this W ork Plan and are presented in Appendix A. Upon review of the quotes, the costs presented appear to be competitive and reasonable based upon Atlas’s experience with this type of work. The Parties acknowledge and agree that the State is not a party to any contract with the Claimant or the Contractor for reimbursement from the PST Fund in the execution of this Work Plan, except to the extent provided by a contract signed by the Executive Secretary (UST). Instead, the Claimant’s reimbursement is governed by the Utah Underground Storage Tank Act and the Utah Underground Storage Tank Rules. The Contractor is entitled to reimbursement solely under his/her contract with the Claimant. 4. The Parties agree that they will use a subcontracting method consistent with the requirements of R311-207. 5. The Parties agree that, as a condition of performing the work under the work plan, the Contractor shall carry the insurance specified in R311-207-3(c)(4).
