Dear Jeffery Hatch:The Division of Drinking Water (the Division) received a request for an Operating Permit for the North Sage Estates Subdivision from your consultant, Kelly Chappell
with Ensign Engineering on April 17, 2024. The Division issued Plan Approval for this project on February 27, 2024.Our understanding of the project is that a new 19-lot subdivision has
been completed on the northwest sideof Koosharem Town. The new subdivision connected to the existing drinking watersystem on 650 North at two locations, approximately 50 West and 100
West. This provides alooped pipeline for the proposed subdivision. Approximately 2,300 feet of 8-inch diameter PVCpipeline, 4 fire hydrants, 1 combination air-vacuum valve, 19 service
connections, and 3 isolationvalves have been installed. A hydraulic analysis for this subdivision was included in the Town’swater master plan dated December 2023. The hydraulic analysis
shows that the new subdivision pipelineswill meet the 40-psi minimum pressure during peak day demand.
We have received the following information for the North Sage Estates Subdivision:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the North Sage Estates Subdivisionis hereby issued as constituted
by this letter. You may now place the Subdivision water lines in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
