Project Summary
Proposed number of EVSEs (type and quantity of each) Four Dual-port Level 2 EVSE Units
Actual number of EVSEs (type and quantity of each) Four Dual-port Level 2 EVSE Units

Actual address of EVSE location(s): 220 East Morris Ave South Salt Lake, UT 84105
Bid amount: The initial bid for the project was $95,067, as stated in Quote #1429 on November 20th,
2018. However, the bid was later revised on May 22nd, 2019, and reduced to $63,549. The original
quote was based on the fact that ChargePoint had both gateway and non-gateway stations, with one
parent (GW) station required for every five stations. However, as of now, ChargePoint has a gateway in
all of their CT4K stations, which has updated the costs. On August 3rd, 2021, a new quote, #2050, was
given, which increased the price of EVSE units from $36,226 to $40,068.
In addition, the Fire Department required the installation of Knox Boxes, and South Salt Lake required a
Performance Bond. These requirements increased the installation cost from $27,323 to $33,127 (as seen
in Quote #1429 on November 10th, 2021). Furthermore, an additional cost was incurred to install a
wooden pole and replace the 400 AMP Main Breaker, which increased the installation cost from $33,127
to $38,320 (as shown in Quote #1429 on December 29th, 2021).
A change order was made to add additional bollards, repair the meter, and replace the wires going to
the meter. This increased the installation cost from $38,320 to $47,551 (as stated in Quote #1429 on
November 4th, 2022).
We won't be able to pay for the Payment Bond ($1,685), the repaired meter, or the replaced stolen wire
($6,068), as these aren't eligible costs for the program. The contracted amount for the Assure plan
under the state contract with Lillypad was $2,046 for each EVSE unit. However, the invoice amount was
$2,116 for each EVSE unit, resulting in a total cost of $8,464 instead of the expected $8,184. We only
paid up to the contracted amount, which means we reduced the payment by $280.
The total cost of the project was $87,619, and eligible costs amounted to $79,586.
Invoice amount: Final invoice was for $87,619 for the purchase, installation of four dual-port level 2
EVSE units. This included Payment Bond ($1,685), the repaired meter or replaced stole wire ($6,068)
that are not eligible for reimbursement. The contracted amount for the Assure plan under the state
contract with Lillypad was $2,046 for each EVSE unit. However, the invoice amount was $2,116 for each
EVSE unit, resulting in a total cost of $8,464 instead of the expected $8,184. We only paid up to the
contracted amount, which means we reduced the payment by $280.
The reimbursement amount is $79,586.
