Subject:After-the-Fact Plan Approval/Operating Permit, Mountain View Estates Subdivision (DS001); Marysvale Town, System #16003, File #13635
Dear Karen Christensen:The Division of Drinking Water (the Division) received the plans and specifications for the Mountain View Estates Subdivision from you on March 2, 2024. Written
review comments were provided to you on March 6, 2024. The Division received the revised plans and specifications on April 11, 2024. Your consultant submitted the Operating Permit certification
letter and supporting documentation on April 22, 2024. This project now satisfies the conditions for receiving an after-the-fact plan approval and operating permit.
Our understanding of the project is that a new 16-lot subdivision has been constructed on the northwest end of town. The subdivision included the installation of 2,110 feet of 8-inch
diameter PVC C900 pipe, 8 gate valves, 8 fire hydrants, and 16 individual water service connections. The subdivision is complete and currently has one active residential connection.
We have completed our review of the plans and specifications, stamped, and signed by Jeffery T. Albrecht, P.E., dated February 6, 2019,and find they basically comply with the applicable
portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. The water system hydraulic analysis from the 2022 Marysvale Town Water Master Plan shows that the Mountain
View Estates Subdivision complies with the minimum pressure requirements for peak day demand and peak instantaneous demand. The fire flow available at the Mountain View Estates Subdivision
is estimated at 370 gallons per minute (gpm). We understand that is the typical fire flow capability throughout most of the existing water system. A letter from the local fire authority
was received acknowledging the low fire flow and stating that the Town is working on an improvement project to improve the available fire flows. On this basis, the plans for Mountain
View Estates Subdivisionare hereby approved.
We have received the following Operating Permit information for the Mountain View Estates Subdivision:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an After-the-Fact Operating Permit for the Mountain View Estates Subdivisionis hereby
issued as constituted by this letter.
Please maintain a copy of this letter with your permanent records for future reference. The issuance of this Operating Permit resolves your water system’s deficiency related to “Unapproved
Facility in Service”. An updated IPS report isavailable at https://waterlink.utah.gov. Please contact Jennifer Yee at (385) 515-1501 or jyee@utah.gov for questions related to your water
system’s IPS report or deficiencies.
If you have any questions regarding this Operating Permit, please contact John Chartier, P.E., DEQ Central District Engineer, at (435) 559-1969, or Michael Newberry, P.E., Permitting
and Engineering Manager, at (385) 515-1464.
