On July 12, 2023, I witnessed the sealing procedure for the well referenced above as authorized by
R309-515-6(5)(b). This letter is to certify that the well sealing procedure met the requirements of
Utah Administrative Code (UAC), Rule R309-515-6(6)(i), Well Sealing Techniques and
Requirements, and Rule R655-4-11.4, Surface Seals and Interval Seals.

Well Sealing Description
Description of the Well Sealing Process: Neat cement grout. A truck from Sunroc brough
8 CY of neat cement (15,960 lb of cement with about 6 gallons of water per 94 lb). This
was not enough to fill the annular space. All Wells drilling ended up batching another 77
bags (92.6 lb bags) of neat cement with about 6 gallons of water per bag. Each batch was
pumped through a 2-inch tremie to the bottom of the borehole. Each batch of 13 bags
weighed approximately 15.0 lbs/gallon per batch. 77 bags of cement equate to about 3.58
CY of grout. The total that was pumped ended up at 11.33 CY. It was calculated that the
annular space would take about 3.27 CY of grout. The reason for the large difference is
that during drilling, a sand layer was encountered at around 80 feet below ground surface
and large void space was created when sand caved into the hole.

Issues Observed during Well Sealing
No issues observed. The grout came to the surface.
This well seal certification is not a Plan Approval to construct a public drinking water well
or an Operating Permit to use the well as a public drinking water source. Such approval may
only be granted by the Director of the Division of Drinking Water. A copy of this letter has been
sent to the Director of the Division of Drinking Water.
