Please find attached the Drinking Water Source Protection Plan Updates for both the Well Sources and the Spring Sources for Hyrum City. Your September 13, 2022 e-mail to Kade Maughan of Hyrum City, indicated that the due date for
the Updated DWSPP required for Well #1, Well #3, and Well #4; Hyrum City; Water System #03008 (Source Numbers WS003, WS007, and WS008) is due 90 days from the date of the e-mail, but that the updated source protection plans
for the Spring Sources are not due until 12/31/2023. In an effort to streamline the process and to get the two updates on a coinciding calendar, Hyrum City asked us to help them prepare both of the Updates (Wells and Springs) at this
time. For this reason we are submitting both updates.

Hi Dan,
I'm reviewing Hyrum City's updated DWSP plans, received on November 23, 2022. One issue needs to be addressed before I can concur:
Water source WS003 (Well #1) has many residential properties, and some businesses, located within the protection zones that were not accounted for in the original plan or succeeding updates. These residences/businesses are
considered potential contamination sources (PCSs) and therefore they need to be inventoried and assessed on the DWSP plan. The Division typically considers residential properties as an uncontrolled PCS due to the limited control and
knowledge of how homeowners use, store, and dispose of household hazardous waste, pesticides, and fertilizers. Therefore, a land management strategy, such as public education, needs to be implemented for residential properties.
This can be accomplished by distributing factsheets to homeowners within the protection zones once every update cycle (six years) or by simply including a link on the mailers directing readers to factsheets posted on the system's
website. Please complete the following and resubmit the updated DWSP plan. The attached source protection user's guide has information that may be helpful:
Add residential properties and businesses to section 3 of the updated DWSP plan.
Assess the added PCSs in section 4 as controlled or uncontrolled using one of the four control types (regulatory, negligible quantities, physical controls, best management practices). Residences should be considered uncontrolled.
Describe the management program for uncontrolled PCSs that will be implemented in section 5 (i.e. public education). Mention that the water system will implement land management strategies by the next update due date and will
repeat implementation once every update cycle (six years). Include a copy of the material that will be distributed to residences. Many systems distribute the attached factsheets to meet the public education requirement.
I can provide 90 days from the date of this email to receive a revised updated plan that addresses these issues before a formal disapproval letter is sent and ips points are given.
