(WSP) is pleased to present this Work Plan and Budget (WP&B) for monitoring wells abandonment at the Utah Department of Transportation (UDOT) Station No. 3433 (Site), located at 3652 North 37900 West, Tabiona, Utah. The Site has Facility ID No. 9000145, Release OCL. This WP&B was prepared in response to the Utah Division of Environmental Response and Remediation (DERR) request via email communication on April 25, 2024. The Site is the location of two former underground storage tanks (USTs) that were closed by removal on October 12, 2021. In November 2022, at the request of the DERR, WSP completed a Site investigation by installing four monitoring wells in the vicinity of the former UST basin and collecting soil and groundwater samples to determine the degree and extent of contamination. In November 2023 WSP collected additional groundwater samples from the monitoring wells at the Site. Findings of these investigations were summarized in Site Investigation Report (SIR) dated March 16, 2023 and Groundwater Monitoring Report dated February 6, 2024, respectively. Based on the findings of the SIR and the 2023 Groundwater Monitoring Report, the DERR determined that no further action is required at the Site (DERR document ERRL-0139-24, dated April 10, 2024). For the purposes of properly abandoning the four monitoring wells at the Site, as required in the Utah Rules (R655- 4), WSP will sub-contract a Utah licensed well driller who will perform the following tasks: • Submit the required notification and reporting to the Division of Water Rights. • Plug the wells in place with 30% solids grout from the bottom of the well to the surface via tremie pipe using grout pump. • Complete the surface with concrete. 10876 South River Front Parkway Suite 250 South Jordan, Utah 84095 wsp.com Page | 2 The total estimated cost to complete the monitoring well abandonment is $6,975. Estimated costs detail is provided in Table 1 and include costs for this WP&B preparation, subcontractor procurement, field oversight, documentation and reporting, and submittal of the petroleum trust fund reimbursement package. Subcontractor costs are based on bids received and included in Table 1. Backup documentation for bids received for this WP&B are included as an attachment to this document. WSP is prepared to mobilize to the Site within 15 business days after receiving DERR and SFN approval for the WP&B. Work will be performed on a time and materials basis. Labor rates are based on the DERR Cost Guidelines for Utah Underground Storage Tank Sites, as updated January 2023. No activities will be performed outside the proposed SOW and budget without prior approval from the DERR. Please contact Ehud Ardon at ehud.ardon@wsp.com if you have questions or comments regarding this WP&B. Respectfully submitted, WSP USA Environment & Infrastructure Inc.
