DESCRIPTION OF FACILITY
This minor modification is in response to a letter from BLX Mayflower, LLC (DWQ-2024-001462), received on April 11, 2024, requesting a compliance schedule milestone extension for milestones
found in Part I.C.3.a. of Utah Pollutant Discharge Elimination System (UPDES) Permit No. UT0026140. The Division of Water Quality approves this request. The below compliance schedule
tables have been modified; modified dates have been bolded. This is also reflected in Part I.C.3.a. of the UPDES Permit.
Since this modification is a change to interim compliance dates in a schedule of compliance, that is no more than 120 days after the date specified in the existing permit and does not
interfere with the attainment of the final compliance date, it is considered a Minor Modification. Minor Modifications are not required to follow the procedures in Utah Administrative
Code R317-8-6, including public notice procedures.
