This report is submitted in accordance with Approval Order number DAQE-AN103350075-18, Title V Operating Permit number 3500008001 Condition 11.8.4.k.3 and R307-326-9 (3)(a)(vii). Difficult to Monitor (DTM) components are inspected annually using Method 21. Tesoro has designated a total of 532 DTM components throughout the various process units. ln 2023, all operating DTM components were monitored. Please refer to Appendix A and Appendix B of this report. Unsafe to Monitor (UTM) components are inspected when it is safe to do so. Cunently Tesoro has designated a total of 22 UTM components. These are located in the Number 2 Grude Unit (N2C) on the V-102 Fractionator Tower overhead relief system. These components are approximately 40' above grade requiring extensive scaffolding. Tesoro will monitor the N2C components at a minimum of every five years with the mandatory API 576 testing on the RV's or eadier if scaffolding is erected for maintenance purposes. Please refer to Appendix A and Appendix C of this report.
