Dear Mr. Hansen:
Pursuant to the Division of Waste Management and Radiation Control ("DWMRC") letter dated
November 9, 2022, and in accordance with notice provided to DWMRC on February 23, 2023, Energy
Fuels Resources (USA) Inc. ("EFRI") attempted the installation of MW-41A the week of March 13, 2023.
The installation of MW-41A was unsuccessful due to the borehole collapsing after drilling during casing
activities. The borehole was abandoned in accordance with applicable Utah Department of Natural
Resources, Division of Water Rights regulations.
EFRI will install an additional well, MW-4 lB approximately 15 feet south of the MW-4 lA location.
EFRI is providing the DWMRC with at least fifteen days notice for installation of groundwater well MW-
41B. EFRI plans to begin field activities starting April 11, 2023.
Installation activities will be completed in accordance with the GWDP and the applicable Utah
Department of Natural Resources, Division of Water Rights regulations. An As-Built Report will be
submitted within 60 days of well completion.
