Box 144820 Salt Lake City, UT 84114-4820 Dear Mr. Gilgen & Mr. Kijowski: Re: Compliance Advisory-Mountain View Mushrooms, Approval Order (AO) DAOE-1027-92 dated October 29, 1992, Millard County. Mountain View Mushrooms has received the compliance advisory from your office dated December 19th, 2023, and intends to comply with the regulations outlined in the UAC. We have recently reached out to Alan Humpherys. the boiler permitting manager to enquire about information regarding updating our boiler permit to reflect our current boiler usage. Our current use of our boiler system is the following: Our Cleaver Brooks, 600 HP Natural gas-fired boiler is no longer in use. We have been currently using the Cleaver Brooks Model CP-200-300 oil/gas-fired boiler with only natural gas. We want to re-register our broilers to indicate this operation. We also have a Sellers Boiler model number 100HP 150SXID. We know that the registration needs to be corrected and need further information on how to get this done. We are currently in the process of getting registration in line with our use and are awaiting further information and inspection. Thank you for your assistance in this matter.
