Box 144840 Salt Lake City, Utah 84114-4840 Initial Subsurface Investigation Report K & C Trading Post (Sinclair) 161 East Main St., Bluff, UT Facility ID #5000084, Release Site OIV Underwood Environmental Consulting, Inc. i EXECUTIVE SUMMARY Underwood Environmental Consulting, Inc. conducted a subsurface investigation for Mr. Vance Nielson (JVP, Inc) owner of the K & C Trading Post. The project site, located at 161 East Main Street, Bluff, Utah, is a retail fueling facility and convenience store. This subsurface investigation was conducted to assess subsurface conditions following the reported release from a leaking underground storage tank (UST). On September 18, 2023 a release was reported by the owner based on a failed automatic tank gauging (ATG) test and confirmed by a certified tank tester. The fueling facility is the only operating retail gas station in the town of Bluff. The release (OIV) is 100% eligible for Petroleum Storage Tank (PST) Fund reimbursement. This report summarizes the results of a subsurface investigation performed in February of 2024. Based on field observations and the analytical results of soil and groundwater samples, the following conclusions are presented: • Five soil borings and four groundwater monitoring wells were installed across the site in February 2024. • The native soil types encountered beneath the surface included sand, clayey silts and sands, clay, sand and gravely sands to the depth explored (25 feet). A four to five foot thick clay zone below the 10 foot depth was underlain by sand and gravelly sand. • Groundwater was observed at approximately 18.58 to 19.99 feet below top-of-casing (TOC) in the four newly installed monitoring wells. The groundwater flow direction was determined to be southwest. The groundwater gradient was estimated at 0.001 ft/ft. • Soil samples collected from two boring locations (MW-1 and MW-3) exceeded Tier 1 screening levels for petroleum constituents. • Groundwater samples were collected from the developed monitoring wells. One well (MW-3) contained 0.65 ft of gasoline. Well MW-1 exceeded Tier 1 for Benzene and Toluene while MW-2 and MW-4 exceeded ISL’s for Benzene. • Two artesian wells are located on site. One is currently in use while the other is temporally shut off at the well head. • Additional subsurface investigation is necessary to delineate the extents of impacts. Based on the findings presented above, UEC recommends additional investigation and monitoring well installation followed by a high vacuum extraction event to remove free product (gasoline) from the groundwater. (UEC) conducted a subsurface investigation for K and C Trading Post (Sinclair) in Bluff, Utah on February 15-16th, 2024. The project site, a retail fueling station, convenience store and hotel is located at 161 East Main Street, Bluff, Utah. The underground fueling system consisted of a 15,000-gallon fiberglass two-compartment gasoline underground storage tank (UST) connected to two dispensers. In September 2023 the automatic tank gauging system indicated a leak in the 5,000-gallon premium gasoline compartment. The facility owner contacted a tank tester who confirmed the leak. The tank was emptied and a release was reported to the Utah Division of Environmental Response and Remediation (DERR). A new replacement UST was ordered while the 10,000-gallon regular unleaded gasoline tank compartment remained in service. The 10,000 gallon unleaded tank compartment had passed the tank tightness test. The subsurface investigation was conducted under approved work plan OIV-01. The site is 100% eligible for Petroleum Storage Tank (PST) Fund reimbursement. The DERR identifies the project site as Facility ID #5000084, Release Site OIV. This report summarizes the results of a subsurface investigation performed on February 15-16, 2024. 2.0 SITE DESCRIPTION The project site is located on the south side of Main Street (Hwy 191) in the town of Bluff east of Cottonwood Wash and north of the San Juan River (Figure 1). Commercial businesses are located across Main Street to the north and a hotel is located on the adjacent property to the west. The site includes a retail gasoline fueling station (Sinclair), a convenience store (K & C store) and a hotel (Kokopelli Inn). The fueling system consist of a two-compartment fiberglass UST containing a 10,000 gallon unleaded gasoline compartment and a 5,000 gallon premium gasoline compartment. The UST is connected to two fueling dispensers under a canopy structure. A 2,000-gallon diesel aboveground storage tank (AST) is located southeast of the UST system. Groundcover consists of asphalt pavement, concrete, gravel, and landscaping. Utilities border the site to the north along Main Street (water, phone/communications and overhead power). Culinary (City) water connects from the water main to the Kokopelli Hotel and to the Store. There are no public sewer or natural gas services in the town of Bluff. A septic system and propane tank are located south of the store and hotel. The project site is essentially flat and slopes gently to the south and is at an elevation of approximately 4,324 feet above mean sea level. Two artesian wells are located on site under water rights number 09-196. Well #1 is located north of the Kokopelli Inn hotel and Well #2 is located east of the K & C Store near the adjacent Recapture Inn (Figure 3). The wells were apparently installed in the 1950s and improved in the 1980s. Well #1 is 8-inch diameter and 700 feet deep and Well #2 is 6-inch diameter and 590 feet deep. In 1981 Well #1 was cased to 63 feet and in 1980 Well #2 was cased to 60 feet according to drillers reports (Appendix A). Initial Subsurface Investigation Report K & C Trading Post (Sinclair) 161 East Main St., Bluff, UT Facility ID #5000084, Release Site OIV Underwood Environmental Consulting, Inc. 5 3.0 TIER 1 CRITERIA Tier 1 Criteria describes the environmental features and possible risks to human health or the environment. The following site specific information is provided: The current land use is commercial. The surrounding properties are commercial (Figure 2). The native soil types encountered during soil borings and installation of monitoring wells included sand, clayey silts and sands, clay, sand and gravely sands to the depth explored (25 feet). The subsurface logs are attached in Appendix B. Groundwater was observed at approximately 18.58 to 19.99 feet below top-of-casing (TOC) in the four newly installed monitoring wells. The groundwater flow direction was determined to be southwest. The groundwater gradient was estimated at 0.001 ft/ft (Figure 4). Distance of petroleum contamination to the nearest underground utility: Underground utilities servicing the site structures enter from the north (water). The facility uses aboveground propane so there is no buried natural gas line entering the site. There is no public sewer system connected to the site which uses septic systems south of the store and hotel. Monitoring well MW-3 is located approximately 50 feet south of the water main located south of Main Street. The UST is located about 25 feet from the water main. Distance of petroleum contamination to the nearest property line and building: Monitoring well MW-4 is located approximately 10 feet from the store. MW-3 is located approximately 50 feet from the store and 50 feet from the north property line. The distance from the UST to the north property line (highway 191 right of way) is about 25 feet. A Utah Division of Water Rights (DWR) point-of-diversion plot indicates several points of diversion in the vicinity of the subject site. Two artesian wells are located on the site (Figure 3). The wells are listed under water rights number 09-196 and designated as well #1 (west side of property) and well #2 (east side of the property). Well #1 is about 112 feet from MW-3 (free product well) and well #2 is about 110 feet from MW-1. Water rights maps, drillers reports and related documents are attached in Appendix A. 4.0 NATURE OF RELEASE AND ABATEMENT MEASURES Release OIV was opened based upon a failed ATG test and confirmed by a tank tightness test. In September 2023 the premium gasoline tank compartment’s ATG system indicated a loss of fuel. The facility owner promptly notified his tank tester. The release was confirmed by a tightness test and the tank compartment was immediately emptied by the fuel distributor. The tank tester then retested the emptied tank compartment and reported that water was apparently entering the tank when a vacuum was applied. The tank tester further indicated the leak was slow and likely in the bottom portion of the tank. The regular unleaded tank compartment was also tested and no leak was detected. Inventory records initially suggested a loss of less than 500 gallons of fuel. The facility owner subsequently placed an order to purchase a replacement tank. Plans were formulated to remove the existing 2-compartment single walled fiberglass UST immediately prior to delivery of a new double walled UST of the Initial Subsurface Investigation Report K & C Trading Post (Sinclair) 161 East Main St., Bluff, UT Facility ID #5000084, Release Site OIV Underwood Environmental Consulting, Inc. 6 same capacity and configuration. The K & C Sinclair station is the only operating retail gas station in the town of Bluff and every effort was made to minimize the down time and get a new fuel system installed and operational. The existing UST was removed on March 6th, 2024 and an abatement was immediately performed in which impacted soils and backfill material were excavated. Details of abatement activities are presented in the pending Abatement Report. A new UST was delivered on March 13th, 2024 and installation of the fueling system commenced. 5.0 METHODOLOGY 5.1 SOIL BORINGS AND MONITORING WELL INSTALLATION Five soil borings were installed and four were converted to 2-inch diameter groundwater monitoring wells (MW- 1 through MW-4) on February 15-16th, 2024 at the project site (Figure 3). Direct push drilling methods were used for boring and well installation. Monitoring well locations were based on data from historical records of nearby leaking UST projects and other geological parameters. Research from prior subsurface investigations suggested a groundwater flow direction to the southwest. Borings for monitoring wells were targeted for a depth of 25 feet. The larger 3.5-inch diameter tooling was used to accommodate 2-inch wells and collect continuous core samples to the depth of each boring. Wells were set at depths ranging from approximately 23 – 25 feet below top of casing. Soil only boring SB-1 was intended for waste characterization and placed as near to the leaking tank as feasible. Static water level measured in wells ranged from 18.58 to 19.99 feet below top of casing. The wells were screened across the water table, developed and surveyed by differential leveling to establish vertical control. The monitoring well construction details are depicted on the subsurface logs provided in Appendix B. 5.2 SOIL SAMPLING All five boring/well locations were “daylighted” using water-knife soil vacuum techniques to five feet below grade prior to drilling. Soil samples were collected in a continuous fashion to the total depth of each boring as drilling progressed. Multiple soil samples were collected from each boring and retained in laboratory-supplied glass jars, labeled appropriately, and placed on ice. The soil samples were analyzed by Chemtech-Ford Laboratories for methyl tertiary-butyl ether (MTBE), benzene, toluene, ethylbenzene, xylenes, naphthalene (BTEXN), total petroleum hydrocarbons – gasoline range organics (TPH-GRO), and TPH – diesel range organics (TPH-DRO) using EPA methods 8260B and 8015C. The composite sample from SB-1 @ 18-20 feet was analyzed for BTEXN/DRO/GRO, Total Lead, Flashpoint and pH. Chain of custody documentation was maintained for all samples from the time of collection until delivered to the laboratory. Field screening of soil sample intervals was conducted by headspace analysis using a photo-ionization detector (PID). The samples were field screened using a Mini RAE 3000 PID. The exploratory borings were logged in the field by a UEC professional geologist using the unified soil classification (USC) system. Initial Subsurface Investigation Report K & C Trading Post (Sinclair) 161 East Main St., Bluff, UT Facility ID #5000084, Release Site OIV Underwood Environmental Consulting, Inc. 7 5.3 GROUNDWATER SAMPLING On February 24th, 2024 water levels were measured in the four new monitoring wells using an electronic water level indicator. Three of the four new wells were subsequently purged of three casing volumes of water prior to sampling. Well MW-3 contained 0.65 feet of free product and was therefore not sampled. A separate disposable bailer was utilized for sample collection at each well. The groundwater samples were labeled appropriately, placed on ice, and submitted to the laboratory under chain-of-custody. The groundwater samples were analyzed for MTBE, BTEXN, TPH-GRO, and TPH-DRO using EPA methods 8260B and 8015C. During the groundwater sampling event the east artesian well (Well #2) was also sampled at the hydrant near the store and included in the same sample set. 6.0 RESULTS 6.1 SUBSURFACE LITHOLOGY The upper five feet of soil was daylighted. These upper soils appeared to be primarily silty/sandy clay with some coarse gravel. Soils below five feet were logged and consisted of fine sand and silty sand interbedded with silt and sandy silt. A clay zone four to five thick was contacted in each boring below the ten-foot level. The clay was wet and soft in the upper portion, stiff in the center and soft in the lower portion. The clay became stiff at about 13-14 feet. Below the clay at the 15-foot level a clean fine – medium sand was contacted which coarsened downwards. The sand became wet at 19-20 feet. Wet gravelly sands were observed below 20 feet. Apparent hydrocarbon odor was observed at the 10-12 foot level and the 20 foot level in select borings. Very little staining was observed in any borings. Subsurface lithology was consistent with fluvial or stream deposits as expected given the proximity of the San Juan River and Cottonwood Wash (Figure 1). The subsurface logs are provided as an attachment in Appendix B. 6.2 FIELD SCREENING Field screening of soil was conducted at variable intervals at each boring location using a PID. The results of field screening are presented on the subsurface logs attached in Appendix B. The field screening generally suggested that hydrocarbon impacts were greatest in soil at the 10-12 foot depth and near the water table interface at 19-20 feet. 6.3 SOIL ANALYTICAL RESULTS Four to five soil samples were submitted for laboratory analysis from each monitoring well boring. The results of soil sample analysis from this subsurface investigation are presented on Table 1. The analytical results presented on Table 1 are compared with Initial Screening Levels (ISLs) and Tier 1 Screening Criteria (Tier 1 levels) (DERR, 2005). Initial Subsurface Investigation Report K & C Trading Post (Sinclair) 161 East Main St., Bluff, UT Facility ID #5000084, Release Site OIV Underwood Environmental Consulting, Inc. 8 Of the five drill holes sampled, hydrocarbon concentrations exceeding Tier 1 criteria were detected in two locations (MW-1 and MW-3). The depths of hydrocarbon impacts were generally in the 10-12 foot interval and above the water table at 19-20 feet. Monitoring well boring MW-3 @ 10-12 feet contained the highest concentration of Benzene (1.94 mg/kg). Boring MW-1 contained benzene at 1.81 mg/kg as well as toluene and ethyl-benzene concentrations exceeding Tier 1 levels. The extent of petroleum hydrocarbon impacts in soil are not fully defined in the north, east and west directions. The laboratory analytical reports and chain-of-custody documentation are provided in Appendix C. 6.4 GROUNDWATER FLOW DIRECTION On February 24, 2024 the depth to groundwater ranged from approximately 18.58 to 19.99 feet below top-of- casing (TOC) in the five newly installed monitoring wells (Table 2). Figure 4 depicts a groundwater contour map for the project site on February 24, 2024. Based on the calculated groundwater elevations, the groundwater flow direction is to the southeast. The gradient across the study area was approximately 0.001 ft/ft. 6.5 GROUNDWATER ANALYTICAL RESULTS Three of the four monitoring wells were purged and sampled on February 24, 2024. Well MW-3 contained free product and was not sampled. A summary of groundwater monitoring data is presented on Table 2. The analytical results presented on Table 2 are compared with ISLs and Tier 1 levels (DERR, 2005). All three wells sampled (MW-1, MW-2 and MW-4) exceeded the ISLs for benzene. MW-1 exceeded Tier 1 for benzene and toluene and exceeded ISLs for TPH/GRO (8.47 mg/L). The extents of petroleum hydrocarbon impacts in groundwater are not defined to ISL or Tier 1 concentrations. The laboratory analytical reports and chain-of-custody documentation are provided in the Appendix C. The water sampled collected from the hydrant connected to Well #2 (artesian) was non-detect for petroleum constituents analyzed. 6.6 FREE PRODUCT Free product was observed in monitoring well MW-3 at a thickness of 0.65 feet. 7.0 QUALITY ASSURANCE AND QUALITY CONTROL Soil and groundwater samples collected during subsurface investigation were collected by a State-certified soil and groundwater sampler under the supervision of a State-certified UST Consultant. Proper sampling protocol, utilizing industry recognized standards, was followed during the investigation. Samples were retained in laboratory-supplied containers (4 oz. glass jars or 40 ml vials) and placed on ice until relinquished to the laboratory. Chain of custody records were maintained and are attached along with the complete laboratory analytical reports in Appendix C. The analytical laboratory utilized is certified by the State for the analyses performed and has quality assurance / quality control procedures in place to ensure the accuracy of the analytical data provided. Based on the findings presented above, UEC recommends additional investigation and monitoring well installation followed by a high vacuum extraction event to remove free product (gasoline) from the groundwater. 9.0 LIMITATIONS Our services consist of professional opinions from a State-Certified UST Consultant, made in general accordance with locally accepted environmental practices at the time of execution. Environmental samples are obtained from specific locations, utilizing methodologies consistent with the Utah Department of Environmental Quality regulations. Even when field investigative work is performed to an appropriate standard of care, certain site conditions may present difficult assessment problems. Such conditions may include, but are not limited to, complex geologic setting and materials, fate and transport character of petroleum hydrocarbons, physical constraints, presence of utilities overhead and below ground, and the limitations of the assessment tools employed. UEC makes no guarantee; either expressed or implied, that the analytical results accurately represent subsurface conditions throughout the entire project site.
