(ATG) System using Standard Test Mode to monitor the Tank portions of the USTs, with Sensors
for the Piping Sumps. However, the following Problems have been documented in you use of this
system:
a.The Tank Setup reports indicate the "Tilt" has not been entered for these FRP USTs. Since
these are FRP USTs the probes cannot be in the center, since there is a rib in the center of
all FRP USTs. Thus the "Tank Tilt" must be calculated. If the calculations results in a zero,
document the calculations and submit to DERR, if not, enter into the setup and reprint a
copy of the setup report and submit to DERR.
The above information must be submitted to this office by 911712002.If it is not received by the date
specified, we will commence the revocation process of your Certificate of Compliance. Continued non-
compliance will result in the fees on your Certified USTs to be increased from $75 per tank to $150 per tank
and failure to pay the additional fee can result in lapsing of your UST Certificate of Compliance. If you have

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary (UST) may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and federal
UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation (DERR)
performed a compliance review for the underground storage tanks at the above facility. The compliance review,
performed on August 17,1999, indicates you have not satisfied the requirements of the Act and are out of
compliance. To achieve compliance you must do the following:
1. Submit a copy of a current precision line tightness test for the product lines associated with tanks #1
andtlz.
2. Submit a copy of a current annual test for proper operation of the automatic line leak detectors on the
pressurized product lines associated with tanks #1 and. #2.
Please return a copy of this speed memo with the above items to speed processing. If the above information is
submitted to this office by November 22,1999, and srrtrstantial compliance is achieved and maintainecl, no further
action will be taken on this matter. Failure to respond to this request by the date specified will initiate the revocarion

The Utah Underground Storage Tank (USD Act provides that the Executive Secretarv (UST) may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and federal
UST statutes. rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation (DERR)
performed a compiiance review for the underground storage tanks at the above facility. The compliance review,
performed on August t7,1999. indicates you have not satisfied the requirements of the Act and are out of
compliance. To achieve compliance vou musr do the following:
1. Submit a copy of a current precision line tightness test for the product lines associated with tanks #1
and #2.
2. Submit a copy of a current annual test for proper operation of the automatic line leak detectors on the
pressurized product lines associated with tanks #1 and #2.
Please return a copy of this speed memo with the above items to speed processing. If the above information is
submitted to this office by November 22.1999, and substantial compliance is achieved and maintained. no further
action will be taken on this matter. Failure to respond to this request by the date specified rvill initiate the revocation

Re: December 22,1998 upgrade deadline for Underground Storage Tanks
Division of Environmental Response and Remediation (DERR) records show you have one or more underground storage tanks
(USTs) which have not met the December 22, 1998 UST upgrade requirements. These tanks are located at WEBER SiAfg
UNIVERSITY, 3750 HARRISON BLVD
Under Federal regulations issued in 1988, UST owners and operators have until December 22, lgg1 to upgrade, replace, or
permanently close USTs that do not meet the requirements for protection against spills, overfills, and corrosion.
This deadline will not be extended. If your tanks are not in compliance by the deadline, they will not be able to receivedeliveries of fuel.
The DERR encourages owners and operators to meet the 1998 upgrade requirements as soon as possible before the December 22,
1998 deadline' It can take several months to upgrade, close or replace an UST system. Delays are not unusual, and contractors
may not be available if you wait. Start planning now!

Any metal flex connectors connected to fiberglass reinforced plastic (FRP) piping at the submersible pump or dispenser must becathodically protected or taken out of contact with the soil.
USTs filled by transfers of less than 25 gallons at a time (usually used oil tanks) are not required to meet the spill and overfillprevention requirements as part of the upgrade requirement.

To continue to maintain a valid Certificate of Compliance, the facility must be in compliance with Federal and
State Rules and Regulations. A review of our records has shown that the following information is lacking:
tl] Submit a properly completed form Notification Form (EPA 7530-1), notifying DERR of the changes
made to the UST systems made as a result of the Installation Permit issued by DERR on February l l,
1998. This form was due within 60 days of completion of the installation, and required to be signed
by the Utah Certified Installer who performed the work.
tzl Submit a copy of a precision line tightness test for the lines replaced on the UST(s), performed since
the new line installation. A test is required to be performed within 30 days of the replacement.
t3l Although the installer indicated the replacement was completed, no Closure Plan was submitted
or approval was given. Submit and obtain approval of a Closure Plan for the replacement of the piping
in the above permit.
t4l Submit a properly completed Closure Notice documenting the proper closure of the old piping for the
USTs.
The above information must be submitted to this office by 5130198.If it is not received by the date specified,
we willcommence the revocation process of yourCertificate of Compliance. Continued non-compliance will
result in the fees on your Certified USTs to be increased from $75 per tank to $150 per tank and failure to pay
the additional fee can result in lapsing of your UST Certificate of Compliance. If you have questions

Dear Underground Storage Tank Owner/Operator:
The Division of Environmental Response and Remediation (DERR)has received notification that one or more
tanks (USTs) (tanks ) will b.@@IDupsraded)at
Utah, facility
ID# lZ(D?Ol regulated USTs can receive fuel and be put into operation, they
must quafry for the Petoleum Storage Tank (PST) Fund and receive a Certificate of Compliance. To qualify
the tanks for the Fund and receive a Certificate of Compliance or maintain your current Certificate of
Compliance, you must do the following:
1. Register the changes in the tanks using EPA forrr 7530-1, Notification for Underground Storage Tanks
(Enclosed). Tanks installed after December 1988 must have the following in place when installed: leak
detection on tanks and piping (including automatic line leak detectors on all pressurized piping), spill and
overfill devices, and corrosion protection. The Utah Certified Tank Installer must sign page 5 of the form
to certify a proper installation.
2. New Tanks or Systems participating in Utah's PST fund Only: Complete the PST Application (yellow) and
Previous Pollution Incidents (green) forms from the enclosed PST Fund application packet. You must
indicate on the PST Application form ttre type of financial assurance you will use for your share of eligible
cleanup costs for each release covered by the Fund, and declare your Financial Responsibility type(s).
3. Conduc{ a tank and line tightness tcst and send a complete copy of the results to the DERR. The tightness
test must be conducted by a Utatr certified UST Tester for any new tank, line or system installed.. A list of
certified UST Testers is included in the PST Fund application packet. All portions of the tank and piping
system which could contain product must be tested. To receive a delivery of fuel for testing, you must
contact DERR for authorization of a one-time delivery for testing purposes for new USTs. Contact the
DERR office at (801) 536-4100, 24 hours in advance, to arrange for this authorization.
4. New Tanks or Systems Only: Pay UST regisfration fees. In addition tanks participating in Utah's PST Fund
must pay new tank PST fees for all new tanks installed. These are: regisnation fee- $150 per tank; New tank
PST Fund fee- $250 per tank. These fees must be paid before a Certificate of Compliance can be issued.
When the above items are completed, send the forms, test results, and any fee payments to the DERR at the
above address. When the necessry information is received, the DERR will issue a Certificate of Compliance
when applicable. Timely completion of the needed forms and other requirements will facilitate the process
and help you avoid loss of the use of your tanks. The Utah Underground Storage Tank Act (Utah Code Ann.
19-6401 et seq.) requires that a Certificate of Compliance be issued for new tanks before they are put into use.
