Please accept this letter and the attached supplemental information as
documentation of site conditions during the September 2005 underground storage
tank closure activities at the above referenced site. Turc steel underground
storage tanks (USTs) u,ere removed at this time. Three USTs remain at the site.
Additionally sampling \Mas performed to evaluate four former dispenser islands and
the associated dispenser piping. A site map is presented as page 5 of this
document.
Based on review of the required soil sampling performed as paft of the UST
closure process, it is my opinion thatthe Utah Department of Environmental
Quality, Division of Environmental Response and Remediation will not require
further action at the site. This opinion is based on the soil sample chemical
analysis results, which are attached to this letter as paft of the previously
referenced UST Closure Notice. All petroleum hydrocarbon concentrations
measured at the site are below the most stringent Utah regulatory action levels

accordance with generally accepted geotechnical and environmental engineering
principles and practices. This rararranty is in lieu of all other warranties either
expressed or implied.
