'd)Lt, DIVISION OF AIH QUALM, Re:Unsafe-To-Monitor Report Pursuant to R3 07-3 26-9 (3 Xa)(vii) Dear Mr. Bird: Pursuant to R307-326-9 (3)(aXvii) and (7), the attached report is for Difficult-to-Monitor (DTM) and Unsafe-to-Monitor (USTM) valves in VOC service at the Silver Eagle Refining-Woods Cross Inc. facility. There are currently 30 DTMruSTM valves in VOC service. There were no leaking DTMruSTM components in 2023. Attached to this report is an annually updated list of valves and components in VOC service that are considered DTM and/or USTM because access requires special fall-protection equipment. Components that are in DTM and USTM service (and are in operation) will be monitored once each year as stipulated in State Rule R307-326-9 (3Xa)(vii). Access to these components will be by manlift, ladders and by accessing overhead pipe ways while using appropriate fall protection equipment.
