Leaking Underground Storage Tank (LUST) Site Closure
Dear Mr. Eggertsen
The case file for this facility has been reviewed b1, your state project manager, who has
recommended that no further corrective action be taken at this time. This no further action applies only to
the specific LUST release EKFD. This recommendation is based upon the information contained in the file
supplied by you or your consultant.
The information you have submitted indicates that any detectable petroleum contamination at the
site is not a threat to human health or the environment as characterized using State Underground Storage
Tank Rules. In the future, if other evidence indicates a spread of contamination from the Facility which may
cause such a threat, further corrective action may be required.

The Underground Storage Tank Act specifies that yearly registration fees and Petroleum Storage
Tank (PST) Fund fees must be received 60 days after the due date. If they are not, the Certificate
of Compliance and./or coverage under the Fund automatically lapses.
Our records indicate that you have not paid your yearly registration fee and interest and/or your Fund
assessments and late penalties for the ,rmount indicated on the attached invoice. To continue to
maintain a valid Certificate and/or Fund Coverage, the past due assessments must be paid by
September 1, 2lX)0. If the past due fees have not been received by September l, 2000, the
Certificate and/or Fund Coverage will automatically lapse.
In the event you allow your Certificate and./or Fund Coverage to lapse, your tank(s) cannot receive
delivery ofproduct or be operated and costs ofinvestigating and cleaning up releases are no longer

Dear Max Eggertson:
The Closure Plan for the above-referenced facility received by the Utah County Health Department
on, March 02,2OOO, has been approved subject to the noted modifications, if any. This
department requires a fee of $10O per tank for the permit to remove or install, and $5O per
upgrade, including one charge each for removal and installation of lines, which can be obtained at
the above address within 72 business hours prior to closure or installation activities. This
department also requires you to schedule our inspector well in advance to be on site at your
closure or installation activities. The local fire department must be notified 72 hours before
beginning closure or installation and may have additional requirements.
The Closure Plan approval is effective for a period of one year from the date of approval. lf
closure does not take place within one year, submittal of a new Closure Plan will be required,
unless otherwise approved by the Executive Secretary (UST).
Any change to an approved Closure Plan must be submitted in writing and approved by our office
before implementation. lf contamination is suspected or found during closure activities, you must
report it to the Division of Environmental Response and Remediation (DERR) at (801) 536-4100
within 24 hours of discovery.
Enclosed is a copy of the "Closure Notice" form which must be completed and submitted to the
Executive Secretary (UST) Kent Gray, lgS0WNorthTemple, Salt Lake City, UT 841 14-4840.
Please submit the environmental and Unified Soil Classification (USC) sample analysis data and
Chain of Custody forms with the Closure Notice as soon as possible, but no later than 90 days
after tank closure.

Dear Steve,fhe underground storage tanks located at Maetex Suppl/
Company, 700 South Universj-ty Ave, Provo, Ut, were precision
tightness tested April 14, L998. The tanks were fu1I systems
tested with the USTest 2000 System using 2 psi of nitrogen for
t,he ullage test and a three hour volummetric underfil-I test.
Test results are enclosed and show the tanks to be tight.
The pressurized fuel lines were hydrostatic tested at 50psi for 30 mi-nutes. Test results show that the Ij-nes are tlght.
The fuel line leak detectors were tested to see if they
would detect a leak of at least three gallons per hour. All leak
deLectors funetion properly.

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary (UST) may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and
federal UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation
(DERR) performed a compliance review for the underground storage tanks at the above facility. The compliance
review, performed on October 19,1994, indicates you have not satisfied the requirements of the Act and are out
of compliance. To achieve compliance you must do the following:
L submit evidence that proper Inventory Control is performed on USTs #7,#2,#3,#4, #5, and #6 by:
Documenting that the liquid volume in each UST is measured, each operating day, to within one-
eighth inch, including measurement before and after each delivery, and reconciled to daily
withdrawals.
The above information must be submitted to this office by 02-03-95. If it is not received by the date specified,
we will commence the revocation process of your Certificate of Compliance. If you have questions concerning

The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary ruSD may revoke a
Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state and
federal UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and Remediation
(DERR) performed a compliance review for the underground storage tanks at the above facility. The compliance
review, performed on October 19,1994, indicates you have not satisfied the requirements of the Act and are out
of compliance. To achieve compliance you must do the following:
l. Submit evidence that proper Inventory Control is performed on USTs #1,#2,#3,#4, #5, and #6by:
a. Documenting that the liquid volume in each UST is measured, each operating day, to within one-
eighth inch, including measurement before and after each delivery, and reconciled to daily
withdrawals.
The above information must be submitted to this office by 02-03-95. If it is not received by the date specified,
we will commence the revocation process of your Certificate of Compliance. If you have questions concerning
