(EFR) documents: 1. EFR, November 13, 2017, Transmittal of 3rd Quarter 2017 Groundwater Monitoring Report Groundwater Quality Discharge Permit UGW370004 White Mesa Uranium Mill. 2. EFR, November 15, 2017, State of Utah Ground Water Discharge Permit No. UGW370004 White Mesa Uranium Mill – Notice Pursuant to Part I.G.1(a). 3. EFR, November 15, 2017, State of Utah Ground Water Discharge Permit No. UGW370004 White Mesa Uranium Mill – Removal of Certain Groundwater Monitoring Parameters from Accelerated Monitoring Status The review was conducted to ensure compliance with all applicable parts of Utah Groundwater Discharge Permit No. UGW370004 (Permit) issued to the White Mesa Uranium Mill. (801) 536-4284 www.deq.utah.gov Printed on 100% recycled paper EFR 3rd Quarter 2017 Groundwater Monitoring Report – Close-out: Based on review of the EFR November 13, 2017 3rd Quarter 2017 Groundwater Monitoring Report, all applicable requirements of the Permit were met and the submitted groundwater monitoring data is reliable. The review is hereby closed out. EFR Notice Pursuant to Part I.G.1(a) – Close-out: Based on review of the EFR November 15, 2017 Notice, all requirements of the Permit were met. The review is hereby closed out. EFR November 15, 2017 Request to Remove Certain Parameters from Accelerated Monitoring Based on review of the EFR November 15, 2017 Request, EFR requests that certain monitoring well parameters be returned to baseline monitoring as listed on the table below: Monitoring Well Parameter Number of Consecutive Sample Results Below the GWCL Baseline Monitoring Frequency MW-15 Selenium 9 Semi-Annually MW-19 Nitrate 9 Semi-Annually MW-29 Sulfate 8 Semi-Annually It was noted that the above monitoring well parameters have had sample results below the Permitted Ground Water Compliance Limits (GWCLs) for the most recent eight or more consecutive accelerated sampling events. In accordance with Division protocols, accelerated monitoring may be discontinued if at least eight consecutive samples show concentrations below the GWCL.
