all applicable requirements of the Permit were met. Therefore, the review is closed out.

EFR Mav 7.2015 Pronosal for Removal of Certain Groundwater Monitorins Parameters from
Accelerated Monitorine Status:
The EFR February 9,2015 submission requests that the ,fbllowing wells/parameters be returned to baseline
monitoring frequency:
o MW-1/Tetrahydrofurano MW-23ffield pH
The request is based on the most recent laboratory resultr; for these wells/parameters showing results less
than the Ground Water Compliance Limits (GWCLs) lisred in the Permit. The Division's review of the
laboratory results verified that the requested wells/paranreters showed eight consecutive monthly laboratory
results below the GWCLs.
Based on this review, the following wells/parameters are, authorized to be returned to baseline monitoring
frequency as follows:
Monitorins Well Parameter Baseline Monitorine Freouencv
MW-l Tetrahvdrofuran Semi-Annual
MW-23 Field pH Semi-Annual
Please note that these wells/parameters are subject to futrure accelerated monitoring requirements in
accordance with Part I.G of the Permit.
EFR Mav 19.2015 Plan and Time Schedule for Monitorine Well IVIW-31
The EFR May 19,2015 Plan and Time Schedule (EFR Plan) addresses exceedances of the approved
recalculated GWCLs for seleniumo sulfate, total dissolved solids and field pH at monitoring well MW-31.
EFR agrees to submit a Source Assessment Report (SAR) for the above listed constituents in monitoring
well MW-31 to the Director within 90 days after approv:rl of the EFR Plan, which will follow the format of
previously submitted SARs and will include information related to all studies performed.
Based on this review, the EFR Plan is hereby approved. Please provide the SAR within 90 calendar days.
