Closeout Letter Dear Ms. Weinel: The Utah Division of Waste Management and Radiation Control (“Division”) has completed the review of the May 9, 2019, 1st Quarter, 2019 Nitrate Monitoring Report for the Nitrate Contamination Investigation. The review was based on compliance with the following: • Energy Fuels Resources (USA) Inc., White Mesa Uranium Mill, Groundwater Discharge Permit No. UGW370004, signed March 19, 2019 (“Permit”). • Corrective Action Plan for Nitrate White Mesa Uranium Mill near Blanding, Utah, May 7, 2012, (“CAP”) approved December 12, 2012. • The EFR Quality Assurance Plan, July 23, 2018, Revision. 7.4 (“QAP”) The Division review concluded that the Report appeared to be in compliance with the Permit, CAP, and QAP.
