Sampling Report - Closeout Letter
Ground Water Quality Discharge Permit Number UGW370004 Dear Ms. Weinel:
The Utah Division of Waste Management and Radiation Control (“Division”) has completed a review of
the 2018 Annual Tailings System Wastewater Sampling Report (“Report”). The review was based on compliance with the following documents:
1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (“QAP”). Revision 7.3,
dated August 18, 2017. 2. Groundwater Discharge Permit No. UGW370004 (“Permit”). Permit version that was signed on January 19, 2018. 3. Tailings and slimes Drain Sampling Program (hereafter “Plan”) Revision 3.0 dated July 8, 2018.

The Division’s review concluded that the Report appeared to be in compliance with the Permit, QAP, and Plan. This letter constitutes a closeout of the 2018 Annual Tailings Systems Wastewater Sampling Report. If you have any questions, please contact Dean Henderson at [phone].
