The Division of Waste Management and Radiation Control has completed its review of the November 27, 2017 3rd Quarter, 2017 Chloroform Monitoring Report (Report). The review was based on compliance with
the following:

1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP), Revision 7.3, dated August 15, 2017. 2. Groundwater Discharge Permit No. UGW370004 (Permit) signed on August 24, 2012.
3. White Mesa Uranium Mill Groundwater Corrective Action Plan (GCAP) Revision 0, dated
September 14, 2015.
The Report complies with the Permit, QAP and GCAP. This letter constitutes a closeout of the Report. If
