Upon Arrival the DRC inspectors briefly discussed the purpose of the site visit inspection with
David Turk of Energy Fuels Resources (USA) Inc. We indicated that we were going to observe
the NESHAP air sampling activity and that the site visit inspection would include a tour of the
Mill.
Inspection Summary
The inspections consisted of a tour of the White Mesa Mill facilities, tailings, and several air
monitoring stations. Due to the Mill being in a temporary stand-by mode, limited work activity
was observed within the Mill's restricted area and no findings were observed.

Inspection Item
General Site Tour: David Turk took the DRC inspectors on a tour of the Mill's facilities
including the ore/feed staging area, processing buildings, warehouse, tailings impoundments and
laboratory. Our tour initially proceeded from the office building to the ore / alternate feed staging
area. Crews were unloading a semi-trailer carrying PACTEC super sacks of alternative feed
material (Dawn Mining alternate feed). One of the containers was observed closely and found to
be triple bagged. The tour proceeded through the various operations of the conventional mill
circuit from the initial grizzly to the solvent extraction, precipitation and drying rooms. Each step
in the Mill was idle allowing several areas to be experiencing active servicing and maintenance.
We observed that the impoundment known as Roberts pond was also still under restoration.
After leaving the Mill buildings we traveled to tailings impoundments Cell 2 and Cell 3 to observe
the on-going NESHAP air sampling activity that began on September 11 and was to conclude on
September 14, 2014. We observed crews working at the staging area for collecting, labeling and
assembling the activated charcoal canisters as well as crews deploying and installing several of
the canisters within the monitoring grid. In addition to observing the NESHAP activity we drove
around the perimeter of tailings impoundments Cell 4A & Cell 4B and visited the borrow site for
interim cover material. It was noted that the borrow material is somewhat coarse grained and it
was discussed with David Turk that a finer grained material would more effectively mitigate
radon emissions.
From the tailings impoundment areas we returned to the Mill buildings and concluded our tour at
the Mill's laboratory. At the conclusion of our tour the Inspectors observed that proper
radiological instrumentation was being used, maintained and calibrated. It was also observed that
all personnel surveyed themselves before leaving the restricted area. Beyond the office building
we traveled with Mr. Turk to observe the installation of a new air monitoring station as well as
several other existing air monitoring stations.
