Notice of Actual Date of Initial Startup of Three Sources
Dear Mr. Rosenblum:
Energy Fuels Resources (USA) Inc. ("EFRI") is the owner/operator of the Arizona 1 underground uranium mine (the
"Mine") located in Mohave County Arizona. Reference is made to the application for construction approval of a
modification for an existing source under 40 CFR 61.07 (the "Application") submitted by EFRI to the U.S
Environmental Protection Agency (the "EPA") on April 2, 2013. EFRI mine personnel discovered additional resources
at the Mine which would bring the total life-of-mine production to over 100,000 tons, and therefore make the Mine
subject to the requirements under 40 CFR 61, Subpart B.
Given that the Mine was operating at the time the Application was submitted, and at the time of the EPA's approval
issued on July 1, 2013, the Application can be considered to serve as EFRI's notice of anticipated date of initial startup,
as required by 40 CFR 61.09(a)(1). Detailed descriptions of the three emissions sources and operations at the Mine were
provided in Section 5.0 of the approved Application.
Please take notice pursuant to 40 CFR 61.09(a)(2) that the actual date of startup of the three emissions sources at the
Mine, approved in your July 1, 2013 letter, occurred on June 26, 2013 when the Mine's resource exceeded 100,000 tons,
and that this notification is being provided within 15 days of actual startup as required.
