Review of the Energy Fuels Resources (USA) Inc. White Mesa Mill Operations
and Maintenance Plan Nitrate Pumping System Revision 1.0.
On January 11, 2013, by email (see Attachment 1 below), the Utah Division of Radiation
Control ("DRC"), received an electronic copy of the Energy Fuels Resources (USA) Inc.
("EFR") White Mesa Mill Operations and Maintenance Plan Nitrate Pumping System Revision
LO ("Plan"). On January 14,2013 DRC received a paper copy ofthe Plan. This Plan was
submitted as required in Section 7.2.2 of the Corrective Action Plan for Nitrate, White Mesa
Uranium Mill ("CAP"). Below are the findings ofthe DRC review of the Plan.
PLAN REVIEW
The Order was signed and executed on December 12,2013. In Table 1 titled Nitrate Corrective
Action Schedule, of the CAP requires the Plan to be submitted by EFR within 30 days ofthe
approved Order. As stated above, DRC received the Plan by email January 11,2013 and
therefore, the Plan was received on time and meet the requirement in Part E.4 of the Order.
Modifications to the Plan
The following sections ofthe Plan need to be modified:
1. In the second sentence in section 2.1 add the following wording (added wording
underlined):
Pumped water will be disposed in the tailings^cells or maybe used for the Mill process
water.
2. Add a section that defines the pumping system with the following wording:

Definition of Pumping System:
• Structure of pumping well including: well casing, well slotted screen casing,
sand/gravel pack, bentonite seal, cement grout seal, protective steal surface casing;
• Pump;
• All structural materials that hold pump in place;
• Electricalpower for the pump and well head heat lamps;
• Electrical wiring to pump and well head heat lamps;
• All hosing, piping, and associated hose and pipe fittings from the pump, including
sampling ports, to discharge hose at tailing cells;
• Temporary water storage tanks;
• Protective boxes over wellheads lined with insulating foam.
The reminder of the Plan appeared to be acceptable for the operations and maintenance nitrate
pumping system. ,
