The Utah Division of Radiation Control has completed the review of 2"** Quarter, 2012 Chloroform
Momtonng Report (hereafter "Report") for the Chloroform Contammation Investigation
The review was based on compliance with the Ground Water Quality Discharge Permit Number
UGW370004 (hereafter "Permit") and the DUSA Quality Assurance Plan (hereafter "QAP") The review
found contents of the Report complies with the applicable Permit and QAP
This letter constitutes a closeout of the 2°"* Quarter, 2012 Chloroform Monitonng Report for the
Chloroform Contamination Investigation
