August 2»0S See Reverse far lnstrucli«ns SUBJECT Chloroform Contamination InvesUgation NoUce of ViolaUon and Groundwater CorrecUve AcUon Order Docket No UGW20 01 DUSA Submittal of a Plan and Work Schedule to Drill and Install Well TW4 27 Conditional Approval Letter Dear Mr Frydenlund The Division of RadiaUon Control (hereafter DRC ) has reviewed the June 17 2011 Plan of and Work Schedule (hereafter Plan ) to drill and install chloroform well TW4 27 The submitted Plan is m response to a May 26 2011 Request for Additional InformaUon Letter for a plan of acuon and work schedule regarding installaUon ofa well(s) hydraulically down gradient of wells TW4 4 and TW4 6 DRC reviewed and responded with some modifications to item 2(c) of the Plan in a conditional approval letter on August 15 2011 On August 24 2011 DUSA responded with addiUonal comments on item 2(c) After addiUonal communicaUon by email between DRC and DUSA the following modificaUon to item 2(c) was agreed on [added wording underlined deleted wording is struck out] 2(c) If the ExecuUve Secretary determines, after review of the second Chloroform Monitoring Report that contains the second of the two quarters of data, or at anv time it is determined that the criteria specified m paragraph 1 2 above have not been saUsfied with the addiUon of TW4 27 then Denison will submit withm 30 calendar days of written noUce from the ExecuUve Secretary after submittal of the Quarterly Chloroform Monitoring Report that contains the second of the two quarters of data referred to above a plan and schedule for installaUon of one or more additional chloroform monitoring wells The new welKs) must show that the 70 fig/1 chloroform isoconcentration line remains hydraulically up gradient to the new well(s) and the groundwater contour lines must show the new well(s) are hydraulically down gradient to TW4 4 and TW4 6 Therefore please modify item 2(c) of the Plan with the wording above and send both a hard copy and a electronic copy of the Plan to the ExecuUve Secretary within 7 working days of receipt of this letter Thank you for your cooperaUon UTAH WATER QUALITY BOARD Rusty Lundberg ^ Co Executive Secretary RL/DCH 195 North 1950 West Salt Lake City UT Mailing Address PO Box 144850 SaltLakeCity UT 84114 4850 Telephone (801) 536 4250 Fax (801) 533-4097 TDD (801)536 4414 WW d q t Ilg P t d 0 100% cycl dp pe
