During the phone call, DUSA asked if it would be alright for its contact laboratory to analyze the samples without repercussions from the DRC? After considering DUSA's request, the DRC feels the groundwater samples in the compromised cooler are lost because the chain of custody seal was broken. Therefore, as required by Section 10.1 of the QAP, DUSA should collect replacement samples within 10 days of discovery. Also in the phone call, DUSA disclosed that UPS had opened five coolers in a previous sampling event, in which, DUSA correctly collected replacement samples. In which sampling event did this happen? Please make note of this in the Quarterly Grounciwater Report due on December 1, 2009, and explain how DUSA complied with the Corrective Action Requirements of Section 10.1 of the QAP. DUSA might also want to check with its consultant on ways to prevent this from happening in the future. If you have any questions, please let me know.
