Energy Fuels Resources (USA) Inc (EFR) White Mesa Mill, Blanding, Utah On December 20, 2012, Russ Topham and Phil Goble of the Utah Division of RadiaUon Control (DRC) conducted an inspection at the EFR White Mesa Mill One of the purposes of the mspection was to observe placement and bunal of bulk byproduct matenal generated at in-situ leach facilities pursuant to an lie (2) Byproduct Receipt Notification from EFR, dated December 12, 2012 EFR is allowed to dispose this byproduct material in Tailings Cell 3 only, which is allowed by License Condition 10 5 of the EFR Radioactive Matenals License On December 12, 2012 DRC received notice that EFR would receive eight loads of 1 le (2) byproduct matenal, and intended to dispose of that waste in Cell 3 on December 20, 2012 Seven loads were ultimately received pnor to the commencement of the bunal activity, with the eighth amving pnor to our exit from the facility Additionally, nine loads from the same generator which EFR had intended to bury over the preceding week were buned dunng our inspection We checked in at the White Mesa mill at 7 00 a m on December 20, 2012 to conduct this inspecUon Soon thereafter we met with Mr Gamn Palmer and David Turk, who accompanied us through the inspection this day ( Matenal disposed consisted of soil, with occasional incidental pieces of metal, wood or other debns The matenal disposed represented loads from Smith Ranch in Wyoming, shipped by Cameco resources, Inc, beanng Shipper No 12/12-1 through 12/12-17, and received onsite December 4 (2 loads), December 6 (1 load), December 7 (4 loads), December 11 (3 loads), December 13 (2 loads), December 18 (3 loads), and December 20 (2 loads) The bill of lading and the receipt documentation form for each of these loads appears in the attachments to this report We observed receipt of the load bearing Shipper No 12/12-16, whose am val tnggered our transition from office-based paperwork review to field review The truck (Enviro Care Truck 308) amved beanng a single roll-off container The matenal receipt proceeded according to EFR's written procedure for receipt and 168 North 1950 West • Salt Lake City, UT Maihng Address P O Box 144850 • Salt Lake City, UT 84114-4850 Telephone (801) 536-4250 • Fax (801) 533-4097 'TDD (801) 536^414 www deq utah gov Prmted on 100% recycled paper Page 2 disposal of 11 e (2) matenals with one exception imtial visual inspection of the matenal occurred at the burial site rather than near the scale house Mr Turk explained the deviation from procedure as a response to frequent receipt of containers with doors frozen shut The 1 le (2) waste disposal operation commenced about 8 00 a m in three excavations Excavations extended into the first 12 inches of tailings sand, and exposed solution at approximately the elevation of the top of the tailings deposit Mr Palmer and Mr David Turk surveyed the level at the bottom of the excavations pnor to placement of the bulk waste therein Using a front-end loader, a small dozer and small backhoe-loader unit, crews placed the waste in the excavations and tamped it down to approximately one foot below the top of the excavation Mr Palmer and Mr Turk then surveyed the elevation of the top of the waste, and the crews filled the excavation to the top with soil In this case, the lift thickness of the waste was less than 4', so the waste was placed in a single lift and compacted by wheel walking with the loader Survey data appear as an attachment to this report Fill soil of 1' thickness was placed on top of the waste Using the front-end loader or the dozer, the fill soil was wheel-walked until the surface ceased visibly yielding to the pressure of the wheels Then the crew groomed the surface to match the surrounding grade The number of passes made by the equipment compacting the waste and cover soil equaled or exceeded the requirement specified in the standard operating procedure Dunng document review following the field observation, I paid particular attention to documents detailing honzontal and vertical location of the waste for this bunal event EFR documents honzontal location of all bunals in an AutoCAD file and on a paper plat map, with all waste from a given source receiving interment m a location reserved for that generator Vertical location includes an elevation measurement taken immediately pnor to placement to establish the bottom of the trench, and another measurement on the top of the waste load after placement in the trench but pnor to placement of capping fill Based on the data reviewed, EFR appears compliant with Part 10 5 E (2) of the RML with respect to the elevation of burial It appeared that all provisions of the approved disposal SOP were followed dunng placement and bunal of the bulk waste, except as previously noted I have attached a photo log documenting parts of the bunal operation Attachments Photo log.
