RE: Updated Weekly and Monthly Inspection Forms to Add the New Chloroform Well TW4-42 for the Chloroform and Nitrate Pumping Systems. White Mesa Uranium Mill near Blanding, Utah, Approval Letter Dear Ms. Weinel: The Utah Division of Waste Management and Radiation Control has reviewed Energy Fuels Resources (USA), Inc. (“EFR”) updated weekly and monthly inspection forms to add the new chloroform well TW4-42 for the chloroform and nitrate pumping systems (“Forms”). The proposal Forms were found in the April 17, 2019 EFR letter, “Transmittal of Energy Fuels Resources (USA) Inc. (“EFRI”) Updated Weekly and Monthly Inspection Forms for the Chloroform and Nitrate Pumping Systems at the White Mesa Uranium Mill.”
