Pursuant to the revised State of Utah Groundwater Discharge Permit ("GWDP") dated March 19, 2019, Section
I.H.4, Energy Fuels Resources (USA) Inc. ("EFRI") is submitting this revision to the Groundwater Quality
Assurance Plan ("QAP") to include the collection of Dissolved Oxygen ("DO") during field sampling. In
addition, EFRI has corrected typographical errors and updated well lists throughout the document.
For ease of review we have provided both redline and clean versions of the document.
