Dear Mr. Howard:
Pursuant to the Division of Waste Management and Radiation Control ("DWMRC") letter dated February 5,
2021, Energy Fuels Resources (USA) Inc. ("EFRI") is providing the DWMRC with at least fifteen days notice
for installation of nitrate wells TWN-20 and TWN-21. EFRI plans to begin field activities the week of April
19, 2021.
Installation activities will be completed in accordance with the GWDP and the applicable Utah Department of
Natural Resources, Division of Water Rights regulations.
An As-Built Report will be submitted within 60 days of well completion.
