Howard Director Yc-2o20-0z11zi MEMORANDUM TO: Phil Goble /1/44,2_ 0 FROM: Dean Henderson ////§ /V -2- DATE: November 18, 2020 SUBJECT: Field Inspection and Split Sampling for the 4th Quarter 2020 Chloroform Monitoring Event For the Energy Fuels Resources White Mesa Uranium Mill, near Blanding, Utah This is a Utah Division of Waste Management and Radiation Control ("Division") staff summary on the inspection of the 4th Quarter 2020 chloroform monitoring event ("Monitoring Event") on November 10 and 11, 2020 for the Energy Fuels Resources (USA), Inc. ("EFR") White Mesa Uranium Mill, near Blanding, Utah. EFR is required to submit a written report of this Monitoring Event by March 1, 2021. The Monitoring Event was inspected against the following enforcement documents: • White Mesa Uranium Mill Ground Water Quality Assurance Plan ("QAP"). Revision 7.5, dated June 5, 2019. • Groundwater Corrective Action Plan for Nitrate White Mesa Uranium Mill near Blanding Utah ("GCAP'), Revision 0, dated September 14, 2015. Summary of Inspection On November 10 and 11, 2020 the Division staff Dean Henderson observed the EFR staff preform groundwater monitoring activities including the collection of water samples from all the chloroform monitor wells TW4-6, 8, 9, 10, 16, 18, 26, 29, 30, and 33. In addition, the Division staff collected a split water sample from monitor wells TW4-6, 8, 9, 16, 18, 29, 30, and 33. A duplicate sample was collected at TW4-9 and is identified as TW4-81. See the photographs decontamination pumping equipment and purging activities in Attachment 1 below. After the monitoring event was completed a close out meeting was held with the following attendees: Division staff Dean Henderson and EFR staff Tanner Holliday, Deen Lyman, Garrin Palmer and Terry Slade. (801) 536-4284 www.deq.utah.gov Printed on 100% recycled paper Page 2 Conclusion Based on Division staff observations of EFR staff performing the sampling and monitoring on November 10 and 11, 2020, it appears that EFR appropriately followed the field requirements in the QAP and GCAP. As stated above, EFR is required to submit a written report of this Monitoring Event on March 1, 2021. The Division will also review this report and will include sample analysis for this Monitoring Event as part of that review to complete the inspection of the 4th Quarter 2020 chloroform monitoring event.
