Closeout Letter Dear Ms. Weinel: The Division of Waste Management and Radiation Control (“Division”) has completed the review of the November 12, 2020, 3rd Quarter, 2020 Nitrate Monitoring Report for the Nitrate Contamination Investigation. The review was based on compliance with the following: • Energy Fuels Resources (USA) Inc., White Mesa Uranium Mill, Groundwater Discharge Permit No. UGW370004, signed March 19, 2019 (“Permit”). • Corrective Action Plan for Nitrate White Mesa Uranium Mill near Blanding, Utah, May 7, 2012, (“CAP”) approved December 12, 2012. • The EFR Quality Assurance Plan (“QAP”) June 5, 2019 (Revision. 7.5) The Division review concluded that the Report was found to be in compliance with the Permit, CAP, and QAP.
