the Groundwater Corrective Action Plan included as Attachment 1 to the SCO. Approval.

The Utah Division of Waste Management and Radiation Control (“Division”) has completed the review
of the July 7, 2021 Plan and the proposed location for new chloroform monitoring well TW4-43.

The Division review for the Plan and the proposed location for new well TW4-43 is hereby approved.

Please provide at least a 14 day written notice so that the Division Director or his representative can
observe drilling and well installation activities.
