Closeout Letter, Ground Water Quality Discharge Permit Number UGW370004 Dear Ms. Weinel: The Utah Division of Waste Management and Radiation Control (“Division”) has completed the
review of the November 26, 2018, 3rd Quarter, 2018 Chloroform Monitoring Report (“Report”). The review was based on compliance with the following: 1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (“QAP”). Revision 7.3,
dated August 15, 2017. 2. Groundwater Discharge Permit No. UGW370004 (“Permit”). Permit version that was signed on January 19, 2018.
3. White Mesa Uranium Mill Groundwater Corrective Action Plan (“GCAP”) Revision 0, dated September 14, 2015. The Division’s review concluded that the Report appeared to be in compliance with the Permit, QAP, and GCAP. This letter constitutes a closeout of the November 26, 2018 3rd Quarter, 2018
