Annual Technical Evaluation, White Mesa Mill Tailings Management System
Transmittal of Correction
Dear Mr. Goble:
Energy Fuels Resources (USA), Inc. ("EFRI") submitted on November 27, 2018 the Annual
Technical Evaluation Report (the "Report") of the White Mesa Mill Tailings Management System,
as required by Condition 12.3 of Radioactive Material License UT1900479. The November 27,
2018 Report covered the period from November 2017 through October 2018. The Report stated
in Section 6.6:
"Movement Monitor 25 (MM-25 4B) on Cell 4B had significant increase in elevation. A
random movement like this is typical of a physical disturbance or a report error. Further
investigation will be completed to determine the cause of the change and if further action
is required."
EFRI has completed the proposed investigation of MM-25 4B. EFRI contacted licensed surveyor
Jones & DeMille, to re-check the survey data values. Jones & DeMille subsequently identified a
data entry error in recording the elevation of MM-25 4B, and provided a revised and corrected
survey data form, certified by Professional Land Surveyor Jason Monroe. Based on the corrected
data, there has been no significant increase in elevation for any movement monitor, including MM-
25 4B, and no further action is required.

Two corrected copies of the Survey Data Form for Cell 4B are attached to this transmittal. Please
replace the incorrect copies in the Movement Monitor attachment tab of each of the two Reports
transmitted on November 27, 2018, with these corrected pages.
