Dear Ms. Weinel:

The Division of Waste Management and Radiation Control has completed its review of the February 24,
2015, 4th Quarter 2015 Chloroform Monitoring Report. The review was based on compliance with the
following:
1. White Mesa Uranium Mill Groundwater Corrective Action Plan (GCAP) Revision 0,
dated September 14, 2015.
2. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP) Revision 7.2,
dated June 6, 2012.
The report complies with the GCAP and QAP. This letter constitutes a closeout of the February 24,
2015, 4th Quarter 2015 Chloroform Monitoring Report.
