Dear Mr. Anderson:
Pursuant to Part I.H.2 of the proposed Groundwater Discharge Permit (“GWDP”) UGW370004, Energy Fuels
Resources (USA) Inc. (“EFRI”) is providing the Division of Waste Management and Radiation Control
(“DWMRC”) with at least fifteen days notice for installation of groundwater wells MW-38, MW-39, and MW-
40. EFRI plans to begin field activities the week of February 12, 2018.
Installation activities will be completed in accordance with the GWDP and the applicable Utah Department of
Natural Resources, Division of Water Rights regulations.
