Pursuant to the Energy Fuels Resources (USA) Inc. (“EFRI") Plan and Time Schedule dated January 10, 2018,
EFRI is providing DWMRC with at least fifteen days notice for installation of chloroform wells TW4-40 and
TW4-41. EFRI plans to begin field activities the week of February 12, 2018.
Installation activities will be completed in accordance with the September 14, 2015 Groundwater Corrective
Action Plan (“GCAP”), included as Attachment 1 to the executed Stipulation and Consent Order (“SCO”) and
the applicable Utah Department of Natural Resources, Division of Water Rights regulations.
