Dear Ms. Weinel:
On May 10 and 11,2017, an inspection was conducted at yoru facility by a representative of the
Division of Waste Management and Radiation Control. Observations from the inspection were
discussed at the closeout meeting with White Mesa Uranium Mill staff. The inspection was an
examination of the activities conducted at your facility as they relate to compliance with the Utah
Radiation Control Rules, the license conditions of the RML No. UT1900479 and Federal Regulations.
The inspection consisted of an examination of representative records, interviews of personnel and
observations by the inspector.
Enclosed is the inspection report regarding this inspection for your review. The Division considers this
inspection closed. We appreciate the Mill staffand their cooperation during this inspection.

During the opening meeting, the inspectors discussed the inspection items for the inspection. The
inspectors also discussed an incident involving a roll-off container that was released from the Mill and an
upcoming site tour for an NRC training course. The Mill was not in operation during the inspection except
for the alternate feed circuit. The Mill staff reminded the inspector of the safety requirements for the Mill.

Item 1. Radiological Postings:
Observations: During the Mill Tour the inspector observed the following posted areas:
The lnspector also observed that the postings were placed in visible areas and they were legible.
Deficiencies: None
Item 2. Exit Surveys: The Inspector observed employees survey themselves out of the restricted area and
checked the alpha meter used.
Observations: The Mill uses Alpha scintillators for exit surveys. The lnspector checked the settings of
one of the survey instruments and found that it was set properly. Meters are calibrated every 6 months.
The inspector also reviewed the Daily Efficiency check form and found no deficiencies. The Mill personnel
that were observed performing exit surveys, did the surveys appropriately.
Deficiencies: Serial number not legible on alpha probe. RSO committed to check all probes and etch the
serial numbers onto the probes. The Inspector will check during the next inspection.
Item 3. Radiological Surveys and Monitoring: The Inspector reviewed the weekly surveys done around
the Mill. The Mill was in a production period from August to November 2016. During production periods
routine survey are done more frequently than in non-production periods.
Observations: The inspector reviewed the radiological monitoring documentation performed at the Mill
site. The documentation indicated that:. All areas of the Mill are being survey for radiological conditions;e Surveys indicate that the Mill is being maintained within regulatory radiological parameterso Documentation is legible, complete and using appropriate units; ando "Clean" areas are beine maintained.

The inspector also looked at dosimeter usage. The inspector observed:o Mill personnel were properly wearing their dosimeters on the torso of the their bodies;o Dosimetery was being properly stored in the locker room and administration building; ando Each employee is assigned a dosimeter.

Item 4. EPA Offsite Rule Visit: On May 10,2016, three EPA Representatives visited the Mill for an
Offsite Rule Inspection for the Dawn Mining alternate feed material.
Observations:
The EPA opened the inspection with a discussion on what their visit was to verifo that the Dawn Mining
alternate feed was being properly processed for its uranium content (Note: The Dawn Mining uranium Mill
and mine site is being cleaned up under CERCLA). Therefore, the EPA is authorized to inspect of any
facility where material is being sent offsite for reprocessing or disposal. During the opening meeting the
Mill management discussed:o Ore receiving (Dawn Mining material is considered an ore because it is being processed for its
uranium content);o The Mill's daily, weekly and monthly inspection;o A Mill process overview;o Groundwater and air monitoring locations; ando Tailing cell design and leak detection.
After the opening meeting the EPA toured select locations of the Mill site. The areas of the Mill toured by
the EPA included:o The ore receiving weigh station;o The ore pad;o The alternate feed circuit;
o The tailing cells; ando Monitor Well MW-22
After the tour of the Mill the EPA reviewed documentation associated with the Dawn Mining material. The
documentation reviewed was:. Paper work related to the receipt of the Dawn Mining material
o Billof Ladings
o Tare receipts;
. Daily, weekly and Monthly reports; ando Documentation related to processing Dawn Mining material (i.e. amount received and amount
processed)
After the inspection the EPA personnel had a short closeout meeting in which they thanked the Mill
Management for their time and providing the requested information and they found the Mill satisfactory for
the continued receipt of the Dawn Mining material.
