Closeout Letter Dear Ms. Weinel:

The Division of Waste Management and Radiation Control has completed its review of the November
17, 2016 3rd Quarter 2016 Nitrate Monitoring Report, the February 22, 2016 4th Quarter 2016 Nitrate Monitoring Report for the nitrate contamination investigation. The purpose of the review was to
determine compliance with the:

1. Corrective Action Plan for Nitrate White Mesa Uranium Mill near Blanding, Utah, May 7, 2012,
(CAP) approved December 12, 2012. 2. The EFR Quality Assurance Plan, June 7, 2012, Revision. 7.2 (QAP).

The Division has determined that both Reports are in compliance with the CAP and QAP. This letter
constitutes a closeout of the 3rd Quarter, 2016 Nitrate Monitoring Report and the 4th Quarter, 2016
Nitrate Monitoring Report.
