Topham, P.E. & DATE:September 22, 2016 SUBJECT:Inspection of Construction of Primary Radon Barrier on Tailings Disposal Cell 2. Radioactive Materials License UT 1900479 (License) - Energy Fuels Resources (USA) Inc. (EFR) White Mesa Mill, Blanding, Utah. On August 23, 2016,1 traveled to the White Mesa Mill to perform multiple inspections, including an inspection of placement and compaction of the primary Radon barrier on Tailings Disposal Cell 2, on behalf of the Utah Division of Waste Management and Radiation Control (DWMRC). The construction reflects the as-yet unapproved requirements of Reclamation Plan 5.1. I checked in at the White Mesa Mill at 7:40 AM on August 23, 2016, in company with two other inspectors from the DWMRC who were there for other purposes, and asked to see David Turk, Site Manager for Environmental Health and Safety. Mr. Turk assigned Mr. Steve Hancock to accompany me on this inspection. When 1 arrived at Cell 2, the work crews were already at work. The work crew consists of a manager (Steve Hancock), a construction superintendent, two bulldozers with operators (one in the borrow area and one in the fill area), a loader with operator (in the borrow area), a sheepsfoot roller and road grader with operators (in the fill area), and three off-road haul trucks and a water truck with operators (working both the borrow and the fill areas). 1 walked the areas of Cell 2 that had received fill, observed the placement, conditioning and compaction of the fill, and observed one density test taken on the fill. 1 also reviewed the stockpiles available to the crew, and observed the borrow operation. Field crews were taking material from Stockpile W1 (see attached map) during my inspection. Some material had also been taken from Stockpile W3 to make room for equipment to pass efficiently back and forth through that area. Average daily production was approximately 3,400 yd3 per day as of the time of the inspection. Density tests were taken approximately every 460 yd3, with locations chosen randomly within each lift, and representing the middle 8” if each lift. This has resulted in 274 density tests taken versus 253 required at the rate of one for every 500 yd3 as specified in Rec Plan 5.1. All but a half-dozen tests revealed density meeting or exceeding standard, with the failing areas reworked and retested until achieving a satisfactory result. The results of the testing are not summarized here, since a good summary was provided. That summary appears in the attachment to this report. Finding: EFR appears to have followed applicable provisions of Reclamation Plan 5.1 in constructing the portions of the cover system installed to date. However, since Reclamation Plan 5.1 remains unapproved as of the date of this Memorandum, the licensee is proceeding at his own risk. Recommendation: I recommend closing out the inspection of August 24, 2016. I discussed this inspection with Logan Shumway, the Mill Manager, prior to departing.
