Dear Mr. Anderson:
Pursuant to the Division of Waste Management and Radiation Control (“DWMRC”) letter dated September 19,
2016, Energy Fuels Resources (USA) Inc. (“EFRI”) is providing DWMRC with at least fifteen days notice for
installation of chloroform wells TW4-38 and TW4-39. EFRI plans to begin field activities the week of October
17,2016.
Installation activities will be completed in accordance with the September 14, 2015 Groundwater Corrective
Action Plan (“GCAP”), included as Attachment 1 to the executed Stipulation and Consent Order (“SCO”) and
the applicable Utah Department of Natural Resources, Division of Water Rights regulations.
