Uranium Mill - As-Built Report Pursuant to Part LF.6 of the Permit
Dear Mr. Anderson:
This letter transmits the As-Built Report for Energy Fuels Resources (USA) Inc.’s (“EFRI’s”) piezometer Piez-
03A.
Piez-03A was installed during the week of May 2, 2016. Piez-03 had high pH measurements recorded
quarterly. Cement grout installed above the filter pack and bentonite bore seal in Piez-03 may have leaked into
the filter pack, causing interaction between groundwater and the grout, resulting in the high pH. To address this
pH issue, Piez-03 was abandoned and replaced with Piez-03A.
The enclosed As-Built Report includes the items required for As-Built Reports in the Permit Part I.F.6, and is
being submitted for Piez-03A. ]
