Dear Ms. Weinel:
On April 27,2016, an inspection was conducted at the White Mesa Uranium Mill by a representative of
the Division of Waste Management and Radiation Control. Observations from the inspection were
discussed at the closeout meeting with the Mill staff. The inspection was an examination of the
activities conducted at the Mill as they relate to compliance with the Utah Radiation Control Rules, the
license conditions of the Radioactive Material License No. UT 1900479 and Federal Regulations. The
inspection consisted of an examination of representative records, interviews of personnel and
observations by the inspector.
Enclosed is the inspection report. The Division considers this inspection closed. The Division would
like to thank the Mill stafffor their cooperation r during this inspection.

During the opening meeting, the inspector discussed the inspection items and documentation to be reviewed
during the inspection. The main portion of the Mill was not in operation during this inspection. The Mill
staff reminded the inspectors of the safety requirements for the Mill.

Item l. Documentation Review: The Inspector reviewed applicable documentation.
Observations: The DWMRC inspector reviewed the following documents:o The current Respiratory Protection Manual and confirmed all required respirator protection
procedures are covered in the manual;
o The respirator issuance log;o Medical clearance, fit testing and respirator training documentation; ando Respirator survey forms.
Deficiencies: None
Item 2. Mill Tour: The Inspector walked through the restricted area on a general site tour. The tour
included the ore pad, the mill, the alternate feed circuit and the respirator room.
Observations: The DWMRC inspector also observed:. Ore (both alternate feed and conventional ore) being stockpiled on the ore pad;o OSL badges were appropriately being used by all observed employees at the Mill;
o Continued equipment maintenance and replacement activities throughout the Mill;o Yellowcake being put into drums in the new yellowcake packaging room;r Respirator room was clean and well organized;r Respirators were clean and in good repair;o Fit testing of respirators was performed appropriately; ando Respirator issuance to Mill personnel is performed appropriately.

Item 3. Other: Follow-up on Leaking f le.(2) Shipment
Observations: The DWMRC inspector followed up on a leaking I le.(2) shipment that was reported to the
DWMRC on March 29,2016. The inspector was able to inspect the decontaminated shipping container and
observe that the container had built in vent ports inside the container. The DWMRC passed all information
regarding this leaking shipment to the Nuclear Regulatory Commission (NRC). The company that sent the
leaking shipment to the White Mesa Mill is regulated by the NRC.
