Dear Ms. Weinel:
The Division of Waste Management and Radiation Control has completed its review ofthe Energy Fuels
Resources (USA), Inc. @FR) proposal for the abandonment and replacement activities of Piezometer P-3 in
the September 10, 2015 EFR letter report, Transmittal of Findings of pH Assessment in Piezometer 3, and
Chloroform Wells TW4-26, TW4-30 and TW4-32 at the White Mesa Mill.
The abandonment and replacement activities of Piezometer P-3 are hereby approved, contingent upon the
following:
1) Abandonment activities will be in accordance with current Utah Division of Water Rights
requirements, UAC R655-4-14, Abandonment of Wells and;
2) The new piezometer will be installed in accordance with EFR Groundwater Discharge Permit
Number UGW370004 requirements, Part I.E.4 and F.6.
